Post to the discussion board a planned approach to communicating with someone who speaks another language. What type of questions will you need to ask? Was there any communication issues discussed this week in Shadow Health? How does communication impact the concept of clinical reasoning in nursing? Please discuss the issues completely, citing your sources so that your classmates can reference the information. Include one insight gained this week from your readings or interaction in Shadow Health.

Example Approach
Obtaining an accurate and comprehensive patient history is dependent upon being able to communicate effectively. Effective communication allows the nurse to gain essential information needed to enable the clinician to apply clinical reasoning processes when ascertaining the patient’s presenting condition and severity of illness and then allows the clinician to use that information to establish the differential diagnoses and plan of care (Barratt, 2018). If the clinician is unable to communicate effectively with the patient, important information may be overlooked which can influence the ability of determining an accurate diagnosis. Additionally, effective communication supports shared decision-making regarding treatments and results in greater levels of patient satisfaction (Barratt, 2018). Interviewing a patient that speaks a language that the clinician does not can be particularly challenging. However, it is imperative that steps be taken to overcome the language barrier. A trained interpreter should be utilized if a patient speaks a different language than the clinician (Bickley,2017). Family members should not be relied upon to interpret. Asking a family member to translate may provide incorrect or incomplete patient information and may violate the patient’s confidentiality (Bickley,2017). The qualified interpreter should be asked to translate everything said and be instructed to not summarize or condense the conversation (Bickley,2017). Additionally, it should not be assumed that the patient can read his or her written language. Prior to being given written medical information, the patient should be asked if they are able to read in their spoken language (Bickley,2017).
The activities in Shadow Health this week reviewed patient interview topics that need to be discussed with the patient and how to structure questions to best acquire the needed information and communicate effectively with patients. Patient interview topics that should be addressed include the reason for the visit, the patient’s medical history, the patient’s current medications and supplements, and all allergies. The Shadow Health lesson discussed obtaining this information by using a combination of open-ended questions, yes or no questions, and quantitative questions. The exercises in Shadow Health helped me understand the importance of follow-up questions. For example, it is not enough to establish that a patient has a food allergy. It is also necessary to determine the severity and type of symptoms associated with the allergic reaction, how the patient treats the allergic reaction, and how the patient prevents a reaction from occurring.
