The vulnerable population group I chose was pregnant women in the United States. At the highest risk for health disparities, are pregnant teens, African American women, and women with little to no education (Chesnay and Anderson, 2020). Advancing evidence-based practices to aid this population is difficult because pregnant women are often excluded from research secondary to the ethical dilemma of potentially inflicting greater harm to the fetus or the mother (van der Zande et al., 2017).
My primary health concern for this group is maternal and infant mortality. The goal is for pregnancies to result in healthy infants/children while maintaining the health of the mother (healthypeople.gov). Identifying risk factors by seeking early prenatal care reduces the percentage of avoidable deaths (Demitto et al., 2017). Pender et al. (2015) state that people can change their behavior and improve their overall health driven by their “self-knowledge, self-regulation, decision making, and problem-solving” with the nurse advocating to promote these changes (p. 27).

The theory I would use to promote changes in this vulnerable population would be a middle-range concept theory. Middle-range concepts and theories look at the person holistically and incorporate socioeconomic status, biological factors, the environment, personal views on health, and quality of life (Chesnay and Anderson, 2020). Pender’s health promotion model would aim at identifying barriers such as access to prenatal care, inherited behaviors, and personal perceptions of health (Petiprin, 2020). Pender’s model views the nurse as an influential part of the person’s life and environment (Petirpin, 2020).References

  1. Discuss how low literacy within your vulnerable population group affects the group’s health and how you, as an APRN, can improve their overall health literacy.
  2. How do the three (social, distributive, market) concepts/types of justice affect the health of your vulnerable population? How would you suggest enhancing social justice in advanced practice nursing?
  3. Analyze how you could motivate the vulnerable population that you chose in Week 1 to make changes. What is your primary health concern for this group and what theory might you utilize to encourage them to make behavior changes?

I enjoyed reading your post on this topic as I worked on a Maternal Child floor for three years and the lack of support that this group sees. As I went with mental illness one of the largest things that we see are pregnant mothers who struggle with mental illness. Mental health problems during the antenatal and postnatal periods affect about one-fifth of pregnant women. Mental illness in pregnant and postpartum women has become a public health concern which has gained attention in recent years (Bharti J. (2020)). With the mentally ill being a vulnerable population and we add a pregnant mothers or a post-partum mother with an infant who has limited resources we set them up for failure. One important case that sticks with me is we had a mother struggling with schizophrenia who was a fresh c-section patient and four facilities denied for access for help because of her “critical” state that she was in. With her mental illness, being denied assistance, and involvement of social workers she made comments that it would be better if she just “stepped in front of a car.” Showing that there is a gap in health care when it comes to pregnancies/post-partum mothers and the resources that are available to them to get proper psychiatric help. With improved case identification and access to evidence-based care is needed for individuals with serious mental illness or psychiatric disorders (Egede, L. E., Ruggiero, K. J., & Frueh, B. C. (2020)).
