Community health nurses play an important role in providing treatment to patients residing in different areas. They are able to reach patients in schools, hospitals, community centers and more. They are also able to travel to communities that do not have easy access to health care. Community health nurses are assess the health needs of a community and work with community stakeholders to provide care and the resources that are needed. Understanding the stakeholder’s needs and interests and including them in the program will benefit the program evaluation and the passing and implementation of new healthcare programs within the community. (Centers for Disease Control and Prevention, 2021).They do this by educating the community on different diseases and ways to maintain their health in order to decrease the occurrence of diseases and deaths. Community health nurses are able to use resources from the community to assist with providing care for patients. Working together with different stakeholders in the community is important because it allows these nurses to have more access to people in the community to provide education and treatment. Examples of this being done is by having health screenings at a local church, dispensing medications from a community center, administering immunizations at a local school or even having educational assemblies at a park. The more people that are reached in a community, the healthier the community can be, which makes it important for community health nurses to work with the community to promote health.

The core goal of a community health nurse (CHN) is health promotion and care for a community (Green, 2018). CHN work with directly with a community to provide resources and education for the betterment of their health and well-being. Their work has evolved over the years, from caring for patients with acute and chronic conditions to health promotion in occupational and educational systems (Green, 2018). For instance, in the era of a pandemic, they provide education on prevention and strategies to improve the safety of the community.
The focus on population allows these nurses to partner with community stakeholders (government, non-profit organizations, and businesses) in health promotion. The World Health Organization (2017) indicates that CHNs seek to understand population needs, identify the diverse determinants of health in communities, provide solutions to address health issues, and implement public health plans and policies. They work with schools, workplaces, and households to address complex challenges affecting these populations. In their capacity, they work with health care facilities, the government, and business owners to address health and health-related issues by providing health care resources, financial assistance, and the development of health policies and plans.
According to the Centers for CDC (2018), a community needs assessment provides community leaders with a summary of the local policy, systems, and environmental change strategies in place and helps identify improvement areas. Appraisal of community resources will help determine what is required and available to meet community needs. When identifying areas of improvement, community resources are essential in prevention and treatment. Therefore, appraisal allows the CHNs to know what is available and its role in health promotion. For instance, a boarding facility can serve as therapy resources for individuals and families.

Community resources provide a means to promote health and dealing with community needs. These resources motivate people to

practice health-promoting activities like use of parks to walk and practice regular exercise. Subsidized or free health promoting resources motivate the community residents to practice healthy living. For instance, gyms, parks, social clubs, and health screening kiosks provide access to resources that can help the community improve their health and well-being.

Assessment plays a vital role in nursing. In every aspect of nursing research has proven that an assessment tool is vital to success and patient/community outcomes. In your post you provided information by the “CDC (2018), a community needs assessment provides community leaders with a summary of the local policy, systems, and environmental change strategies in place and helps identify improvement areas”. This information provides the importance of assessment in community health. I have found additional information in the CDC article that you provided. CDC (2018) defined how an assessment is utilized in public health nursing “The Public Health Accreditation board defines community health assessment as a systematic examination of the health status indicators for a given population that is used to identify key problems and assets in a community”. The assessment provides the community health nurse with information on how to improve public health and how to provide proper education and interventions.

Hi Margaret, Great post this week! In your discussion you mention community resources provide a means to promote health. Green states, “Health promotion and care for the community and population at large reflect public/community health nurses’ mission, vision, and commitments.” (Green, 2018) Both statements are true. Utilizing stakeholders helps promote health and also helps guide the nurses within the community and better assess, plan and treat.

Great post, I came across the Community Organization Model which is a process that empowers communities to improve their health. The community identifies health problems, their causes, and solutions to the problem. They engage participation from other groups and organizations within their community and provide constant feedback. Community participation in health programs increases the ability of the program to affect change, ensuring success of the programs.(Rural health information hub.2021).

According to the American Nurses Association, community health nurses encourage lifestyle changes in communities and provide education on healthy living particularly to the most vulnerable society. They work on infection prevention, immunization, environmental health, and opioid crisis response. Nurses are supporting policy, advocating, and educating the population (ANA, n.d.).

A community health is a nurse who focuses on care for acute and chronic conditions outside the hospital setting that primarily involves restorative care, along with the health promotion of individuals and families while providing care in occupation and educational system settings (Green, 2018). Public health nursing is a population-focused practice that “concentrates on the defined population’s needs for prevention of illness and health improvement” (Green, 2018). Thus, these two terms are synonymous for the role of nurses outside the institutional setting, yet still distinct from each other (Green, 2018). All in all, they provide treatment to patients, and offer them health education to jumpstart and maintain healthy lifestyles, which will then reduce sickness and hospitalizations (North-Eastern University, 2021).
These nurses typically collaborate with other disciplines and community stakeholders, who are both involved and directly affected by the plans, actions, and outcomes of population health care (Green, 2018). Many different stakeholders can lead or participate in championing and implementing “community-based solutions for advancing health equity by making it a shared vision and value, increasing the community’s capacity to shape outcomes, and fostering multi-sector collaboration” through effective partnerships (Baciu et al., 2017). These stakeholders can be local government officials, community groups, faith-based organizations, or local business owners (Green, 2018). These resources are important in population health promotion because they are able to deploy unique skills and access resources to serve a variety of roles in community-based solutions for health equity.

For your response post, you do not need references as personal experiences would count as response posts. If you however cite anything from the literature for your response post, it is required that you cite your reference. You should include a minimum of THREE references for papers in this course. Please note that references should be no more than 5 years old except recommended as a resource for the class. Furthermore, for each discussion board question, you need ONE initial substantive response and TWO substantive responses to either your classmates or your instructor for a total of THREE responses. There are TWO discussion questions each week, hence, you need a total minimum of SIX discussion posts for each week. I usually post a discussion question each week.
