Case Study: Cell Biology and Genetics: Listed below are a number of case studies. Your professor will either assign you a case study or ask you to choose one. If no one has covered all of the case studies, your instructor may assign a case study that no one has covered. Make sure to integrate your advanced physiology and pathophysiology knowledge across the lifespan with the clinical implications for the advanced practice nurse.

1. Ensure that all of the case study’s topics have been covered.
2. To support the content, cite at least three sources, such as journal articles, textbooks, or evidence-based websites.
3. All sources must be within the last five years.
4. Don’t use.com, Wikipedia, or up-to-date as your sources.
Case Study No. 1
Marsha and Clement are both carriers of sickle cell disease, an autosomal recessive disease. Amelia, their first child, is disease-free. Marsha and Clement are considering having another child, but they are concerned that their second child will have the condition. Clement’s father died of sickle cell disease complications shortly before Amelia was born.
1. Use a Punnett square to calculate the likelihood of Marsha and Clement having a sickle cell baby. What are the chances that the baby, like the parents, will be a carrier of the disease?
2. Marsha suggested to the nurse at the local family planning clinic that if the baby was a boy, he might be more susceptible to the disease, similar to his grandfather. How would you react if you were this Practitioner?
3. How likely is it that Amelia, who does not have sickle cell disease, will have children with the disease if she grows up and marries someone who does?
Case Study No. 2
Maria is a sedentary, overweight 68-year-old woman. She complains that her hands and feet are always cold, and that cleaning the house exhausts her quickly. Her blood pressure was 184/98 mm Hg at her most recent doctor’s appointment. Maria has edema around her ankles and legs, and her doctor is concerned about an echocardiogram that shows her heart is enlarged.
1. Identify two possible causes of tissue ischemia in Maria. How might this result in hypoxia?
2. When tissue cells are hypoxic, what two early and reversible changes occur?
3. What type of cellular adaptation has occurred in Maria’s enlarged heart? What led you to this conclusion?
4. Ascertain the cause of Maria’s heart enlargement. Why is it that this enlargement does not provide her with the same cardiac strength and endurance as a well-trained athlete?
Case Study No. 3
Kevin spent ten years working in a uranium mine, excavating uranium for a nearby nuclear power plant. He was diagnosed with small cell lung cancer 25 years later. Kevin is anorexic and has significantly lost weight. His muscles are wasting away, and he is becoming frail. He tries to move around the house throughout the day, but he quickly tires. It has been difficult for him to obtain care, and his cancer treatment is only beginning.
1. DNA damage occurred as a result of ongoing ionizing radiation exposure. Outline the three stages of carcinogenesis that occurred following his radiation exposure.
2. Kevin is normally a fit and active man, and his wife frequently commented on how much food he ate after a day at mine. Why would muscle wasting and weight loss occur now? Explain your response using your

4. Muscle weakness in some cancer patients may be caused by the production of onconeural antigens. Describe how these antigens work. In Kevin’s case, what shape would this process most likely take?
4th Case Study
Felicity, a 29-year-old professional woman, is extremely busy. She has type 2 diabetes and is pregnant for the first time. Due to her hectic schedule, it took her three weeks to visit the family doctor to confirm her pregnancy. Felicity became concerned when her doctor inquired about her use of folic acid. It was all Felicity could do to remember to manage her insulin levels, and she hadn’t even considered taking folic acid supplements. Felicity’s doctor advised her to take 600 g of folic acid daily and to return later for maternal serum marker testing.
1. Discuss the teratogenic effects of folic acid deficiency on the developing fetus. What else stands out as a risk factor in Felicity’s case?
2. What are the advantages of maternal serum marker testing? In this situation, what other test would be especially useful for monitoring the development of Felicity’s baby?
3. When and why is the fetus most vulnerable to the effects of teratogens?

I strongly encourage that you do not wait until the very last minute to complete your assignments. Your assignments in weeks 4 and 5 require early planning as you would need to present a teaching plan and interview a community health provider. I advise you look at the requirements for these assignments at the beginning of the course and plan accordingly. I have posted the YouTube link that explains all the class assignments in detail. It is required that you watch this 32-minute video as the assignments from week 3 through 5 require that you follow the instructions to the letter to succeed. Your initial discussion post should be a minimum of 200 words and response posts should be a minimum of 150 words. Be advised that I grade based on quality and not necessarily the number of words you post. A minimum of TWO references should be used for your initial post. For your response post, you do not need references as personal experiences would count as response posts. If you however cite anything from the literature for your response post, it is required that you cite your reference. You should include a minimum of THREE references for papers in this course. Please note that references should be no more than 5 years old except recommended as a resource for the class. Furthermore, for each discussion board question, you need ONE initial substantive response and TWO substantive responses to either your classmates or your instructor for a total of THREE responses. There are TWO discussion questions each week, hence, you need a total minimum of SIX discussion posts for each week. I usually post a discussion question each week. You could also respond to these as it would count towards your required SIX discussion posts for the week.
I understand this is a lot of information to cover in 5 weeks, however, the Bible says in Philippians 4:13 that we can do all things through Christ that strengthens us. Even in times like this, we are encouraged by God’s word that we have that ability in us to succeed with His strength. Important information on Writing a Discussion Question
- Your response needs to be a minimum of 150 words (not including your list of references)
- There needs to be at least TWO references with ONE being a peer reviewed professional journal article.
- Include in-text citations in your response
- Do not include quotes—instead summarize and paraphrase the information
- Follow APA-7th edition
- Points will be deducted if the above is not followed
Participation –replies to your classmates or instructor
- A minimum of 6 responses per week, on at least 3 days of the week.
- Each response needs at least ONE reference with citations—best if it is a peer reviewed journal article
- Each response needs to be at least 75 words in length (does not include your list of references)
- Responses need to be substantive by bringing information to the discussion or further enhance the discussion. Responses of “I agree” or “great post” does not count for the word count.
- Follow APA 7th edition
- Points will be deducted if the above is not followed
