Hello class,
With this week’s assessment of cardiac, peripheral vascular and respiratory systems, I came to realize that, although I work in an intensive care unit (ICU), I still need to practice with my identifying of S3, S4, murmurs and extra sounds like a mitral valve prolapse. One of the most difficult problems I’ve had to overcome have been these skills.
To master theses requires repetition, patience and lots of practice, (Bickley, 2017). In order to perform an accurate diagnosis, one needs to perform detailed assessments, investigations, and other diagnostic tests must be used in order to assist in providing an answer (Dains, Baumann, & Scheibel,).
Other resources I have used to practice on an assessment of respiratory and cardiovascular focused problems has been: Profession Skill Builder/ Heart Sounds & Murmur library. The link is . It has made it easier to practice health assessments, there are pictures and actual videos to assist in accurate performances.
The Shadow Health modules have helped me increase my proficiency with practice. I am constantly forgetting the correct questions to ask and realize I must practice repetition in order to become proficient in health assessments. As this item is an ongoing skill building process that I must focus on, I believe that Shadow Health allows one to practice not only the assessment, but the health history to allow one to sharpen ones skills.

As an ICU nurse myself, I also struggle with the identification of murmurs. Unless I have a patient after patient coming in with heart murmurs, it is few and far between getting a cardiac patient. I think this would be different if I were a cardiac nurse.
I can hear quite a few murmurs when I work in the NICU of issues relating to an unclosed PDA. I do remember a mnemonic one of my nursing professors told us to remember murmurs by:
RILE (right inspiration, Left expiration). Right-side heart murmurs occur during inspiration, and left side murmurs occur during expiration.
Early detection and accurately being able to identify a murmur is “just as important as gathering a patient’s history” (Thomas et al., 2020). As healthcare providers, the earlier we can detect a murmur and the etiologies associated with them, we will reduce the morbidity and mortality of patients (Thomas et al., 2020).

Hello Robert,
I was going through discussions trying to find someone who had a little difficulty with murmurs as well. I must say that it does in fact take a lot fo practice and repetition to properly identity murmurs. But, I encourage you to keep on practicing. For it is vital to hear out for murmurs since they are early detection signs! ( Thomas, 2020)
