Your AIMS statement doesn’t quite work–I don’t under stand the 70% that you have in the statement. A better statement could be:
In diabetic patients with A1c > 8.0%, case managers will utilize a telehealth program to reduce A1c levels to < 8.0% in 70% of eligible patients over the next 12 months.
The 70% is pretty high. Usually a lower target is recommended such as 30 to 40%.
