NURS-FPX4035: Root-Cause Analysis and Safety Improvement Plan

For this assessment, you must use the supplied template to conduct a root-cause analysis. The completed assessment will be the template, focusing on the specific safety concern identified in your previous assessment.

Introduction

As patient safety concerns continue to be addressed in healthcare settings, nurses can play an active role in implementing safety improvement measures and plans. Often root-cause analyses are conducted and safety improvement plans are created to address sentinel or adverse events such as medication errors, patient falls, wrong-site surgery events, and hospital-acquired infections. Performing a root-cause analysis offers a systematic approach for identifying causes of problems, including process and system-check failures. Once the causes of failures have been determined, a safety improvement plan can be developed to prevent recurrences. The baccalaureate nurse’s role as a leader is to create safety improvement plans as well as disseminate vital information to staff nurses and other healthcare professionals to protect patients and improve outcomes.

As you prepare for this assessment, it would be an excellent choice to complete the Quality and Safety Improvement Plan Knowledge Base activity and to review the various assessment resources, all of which will help you build your knowledge of key concepts and terms related to quality and safety improvement. The terms and concepts will be helpful as you prepare your Root-Cause Analysis and Safety Improvement Plan. Activities are not graded and demonstrate course engagement.

Overview

Nursing practice is governed by healthcare policies and procedures as well as state and national regulations developed to prevent problems. It is critical for nurses to participate in gathering and analyzing data to determine causes of patient safety issues, in solving problems, and in implementing quality improvements.

For this assessment, use the specific safety concern identified in your previous assessment as the subject of a root-cause analysis and safety improvement plan.

Instructions

The purpose of this assessment is to demonstrate your understanding of and ability to analyze a root cause of a specific safety concern in a healthcare setting. You will create a plan to improve the safety of patients related to the safety quality issue presented in your Assessment Supplement PDF in Assessment 1. Based on the results of your analysis, using the literature and professional best practices as well as the existing resources at your chosen healthcare setting, provide a rationale for your plan.

Use the Root-Cause Analysis and Safety Improvement Plan [DOCX] template to complete the assessment.

Additionally, be sure that your plan addresses the following, which corresponds to the grading criteria in the scoring guide. Please study the scoring guide carefully so you understand what is needed for a distinguished score.

  • Analyze the root cause of a patient safety issue or a specific sentinel event in an organization.
  • Apply evidence-based and best-practice strategies to address the safety issue or sentinel event.
  • Create a viable, evidence-based safety improvement plan.
  • Identify existing organizational resources that could be leveraged to improve your plan.
  • Communicate in writing that is clear, logical, and professional, with correct grammar and spelling, using current APA style.

Additional Requirements

  • Length of submission: Use the provided template to create a root-cause analysis and safety improvement plan. There is no page limit. A title page is not required but you must include a reference list as per the template.
  • Number of references: Cite a minimum of 3 sources of scholarly or professional evidence that support your findings and considerations. Resources should be no more than 5 years old. Use the BSN Nursing Program Library Guide as needed.
  • APA formatting: Format references and citations according to current APA style. See the APA Module.

Competencies Measured

By successfully completing this assessment, you will demonstrate your proficiency in the following course competencies and scoring guide criteria:

  • Competency 1: Analyze the elements of a successful quality improvement initiative.
    • Apply evidence-based and best-practice strategies to address a safety issue or sentinel event.
    • Create a feasible, evidence-based safety improvement plan to address a specific patient safety issue.

    • Analyze the root cause of a specific sentinel event or a patient safety issue in an organization.
  • Competency 3: Identify organizational interventions to promote patient safety.
    • Identify existing organizational resources that could be leveraged to improve a plan.
  • Competency 5: Apply professional, scholarly, evidence-based strategies to communicate in a manner that supports safe and effective patient care.
    • Organize content so ideas flow logically with smooth transitions; contains few errors in grammar or punctuation, word choice, and spelling.
    • Apply APA formatting to in-text citations and references exhibiting nearly flawless adherence to APA format.

Comparison of Nursing Stress in Public vs Private Hospitals

Comparison of Nursing Stress in Public vs Private Hospitals

Introduction

The profession of nursing is considered one of the most stressful professions in the world. Nursing is by nature a stressful occupation. Continuous and long-term stress can result in physical, psychological, and behavioral problems in nurses. Therefore these professionals have been researched time and again. To cope with these problems several coping styles are deployed by the nurse which help them deal with the everyday stressors of their life. It has been found via research that using certain unhealthy coping styles may result in a number of mental health problems (Srinivasan & Samuel, 2014).

Nursing is the kind of job in which professionals have to deal with immense workload which can result in a lot of health problems, both physical and psychological. . not only that, the patients that come to hospitals themselves accompany a host of problems with them such as the stress of being ill, being frightful and apprehensive about their future and often they are very bitter and sometimes even indignant. Nurses have to deal with all of this and have to maintain a sense of composure at the same time. Sometimes the nurses have to perform the role of doctors and deal with the attendants and patients which mean they have to take the insults that were meant for the doctors and wasn’t initially a part of their job requirement. Some patients and attendants can even be physically abusive at times; this can result in even more stress for the nurses. It is often seen that at times the patients are way too demanding and expect more than what their job entails, when those expectations are not met the patients can be aggressive and they tend to show their aggression in a both verbal and physical way. These stressors expose nurses to a host of problems, depression, anxiety and insomnia being a few of them (Srinivasan & Samuel, 2014).

What Are SMART Goals for Nursing Practicum?

The practicum experience allows you to develop and expand your advanced nursing knowledge and skills at the aggregate, organizational, or policy level. During week 1 you will submit two to three individual learning objectives that you wish to achieve during this practicum experience.  You should prepare a set of measurable learning objectives, using Bloom’s Taxonomy (See attached file). Your practicum objectives should reflect your interest in expanding knowledge. They are not to include activities related to your DNP project. These objectives will be identified during your mentoring course (NURS 8700P, 8700, 8701).  Practice experiences should be designed achieve specific learning objectives related to the DNP Essentials and specialty competencies (AACN, 2006).

Your practicum objectives should include the following:

1) Three learning objectives using Bloom’s Taxonomy.

2) Description of how these objectives will expand your advanced nursing knowledge and skills at the aggregate, organizational, or policy level.

3) Relationship of the learning objectives to the DNP Essentials.

NOTE: Learning objectives must be submitted using APA style. Points may be deducted for lack of references, writing style, and APA format. Refer to the sample attached in the file area (See attached file >> very important)

Current Issues for the PMHNP

Texas Suicide Trends and State Initiatives

Suicide death rates for Texas in 2022 fell in the range of 11.26 to <16.52 per 100,000 persons, which positions the state in the middle category compared to other states (Stats of the State – Suicide Mortality, n.d.). Although the relatively average suicide death rate in Texas in 2022 caused no initial concern, the state’s overall mental health situation indicates troubling concerns. Texas stands at position 36 in the Mental Health in America Report (2024) due to high rates of mental illness combined with insufficient mental health care availability. Texas holds the last position in the nation by ranking 51st for mental health service access. About 22.9% of adults with any mental illness are uninsured, and 16.5% of privately insured individuals report their insurance does not cover mental or emotional health, both the highest rates in the country (Mental Health America, 2024). Further, statistics indicate that 78.5 percent of adults suffering from substance abuse disorders lacked access to care. The state of Texas has taken multiple measures despite the alarming numbers. School-based programs, including the Texas Suicide Prevention Collaborative and TCHATT (Texas Child Health Access Through Telemedicine), work to boost early diagnosis while improving students’ access to mental health services. The programs show promise for better results, yet expansion is necessary to serve all populations in need.

The PMHNP’s Role in Policy and Advocacy

The combination of firsthand mental healthcare expertise and broad knowledge of mental health systems gives Psychiatric Mental Health Nurse Practitioners (PMHNPs) the ability to develop significant policy changes. Texas residents need PMHNPs to lead policy initiatives because mental healthcare accessibility is currently restricted, and these professionals work to expand practice autonomy and improve insurance benefits. Texas PMMNPs must back the expansion of Medicaid coverage to cut down on both rates of uninsured patients and the costs of treatment. The power of legislative progress increases when psychiatric mental health nurse practitioners actively join policy bodies and public forums, and the American Psychiatric Nurses Association (Kumar et al., 2020). The health of patients and health systems requires nurses to push for better telepsychiatry benefits and reasonable financial compensation. PMHNPs can develop better mental health policies by working with stakeholders alongside their clinical advocacy to enhance treatment results for diverse Texas residents living in both city and suburban areas.

 Building Resilience Through PMHNP Interventions

PMHNPs demonstrate essential functions in developing resilience within both individual patients and communities. The ability to adapt to difficult situations, known as resilience, enables people to handle problems stemming from trauma alongside poverty and discrimination. Mental Health America (2024) reports that Texas youth between 6 and 17 demonstrate wellness to the degree of 60.6%, while 40% exhibit deficient resilience attributes, including self-control and determination. PMHNPs use trauma-informed care together with cognitive-behavioral stress management approaches, incorporating mindfulness to create resilience in their patients. Through their work, PMHNPs educate communities by teaching teachers while delivering mental health intervention programs to schools and guiding students in emotional control practices. PMHNPs connect with local organizations to create family-oriented mental wellness programs that promote student well-being (Schadewald, 2024). Their initiatives enable people to enhance essential social support systems, which become vital during mental health emergencies.

The described expectations meet the passing level of 80%. You are directed to review the Discussion Grading Rubric for criteria which exceed expectations.

Nursing-Leadership Philosophies

Nursing-Leadership Philosophies

Nursing-Leadership Philosophies

Develop and submit a personal leadership philosophy that reflects what you think are characteristics of a good leader.

Use the scholarly resources on leadership you selected to support your philosophy statement.

Your personal leadership philosophy should include the following:

A description of your core values
A personal mission/vision statement
An analysis of your CliftonStrengths Assessment summarizing the results of your profile
A description of two key behaviors that you wish to strengthen
A development plan that explains how you plan to improve upon the two key behaviors you selected and an explanation of how you plan to achieve your personal vision. Be specific and provide examples.

You must proofread your paper. But do not strictly rely on your computer’s spell-checker and grammar-checker; failure to do so indicates a lack of effort on your part and you can expect your grade to suffer accordingly. Papers with numerous misspelled words and grammatical mistakes will be penalized. Read over your paper – in silence and then aloud – before handing it in and make corrections as necessary. Often it is advantageous to have a friend proofread your paper for obvious errors. Handwritten corrections are preferable to uncorrected mistakes.

Use a standard 10 to 12 point (10 to 12 characters per inch) typeface. Smaller or compressed type and papers with small margins or single-spacing are hard to read. It is better to let your essay run over the recommended number of pages than to try to compress it into fewer pages.

Likewise, large type, large margins, large indentations, triple-spacing, increased leading (space between lines), increased kerning (space between letters), and any other such attempts at “padding” to increase the length of a paper are unacceptable, wasteful of trees, and will not fool your professor.

The paper must be neatly formatted, double-spaced with a one-inch margin on the top, bottom, and sides of each page. When submitting hard copy, be sure to use white paper and print out using dark ink. If it is hard to read your essay, it will also be hard to follow your argument.

ADDITIONAL INSTRUCTIONS FOR THE CLASS

Discussion Questions (DQ)

  • Initial responses to the DQ should address all components of the questions asked, include a minimum of one scholarly source, and be at least 250 words.
  • Successful responses are substantive (i.e., add something new to the discussion, engage others in the discussion, well-developed idea) and include at least one scholarly source.
  • One or two sentence responses, simple statements of agreement or “good post,” and responses that are off-topic will not count as substantive. Substantive responses should be at least 150 words.
  • I encourage you to incorporate the readings from the week (as applicable) into your responses.

Weekly Participation

  • Your initial responses to the mandatory DQ do not count toward participation and are graded separately.
  • In addition to the DQ responses, you must post at least one reply to peers (or me) on three separate days, for a total of three replies.
  • Participation posts do not require a scholarly source/citation (unless you cite someone else’s work).
  • Part of your weekly participation includes viewing the weekly announcement and attesting to watching it in the comments. These announcements are made to ensure you understand everything that is due during the week.

APA Format and Writing Quality

  • Familiarize yourself with APA format and practice using it correctly. It is used for most writing assignments for your degree. Visit the Writing Center in the Student Success Center, under the Resources tab in LoudCloud for APA paper templates, citation examples, tips, etc. Points will be deducted for poor use of APA format or absence of APA format (if required).
  • Cite all sources of information! When in doubt, cite the source. Paraphrasing also requires a citation.
  • I highly recommend using the APA Publication Manual, 6th edition.

Use of Direct Quotes

  • I discourage overutilization of direct quotes in DQs and assignments at the Masters’ level and deduct points accordingly.
  • As Masters’ level students, it is important that you be able to critically analyze and interpret information from journal articles and other resources. Simply restating someone else’s words does not demonstrate an understanding of the content or critical analysis of the content.
  • It is best to paraphrase content and cite your source.

LopesWrite Policy

  • For assignments that need to be submitted to LopesWrite, please be sure you have received your report and Similarity Index (SI) percentage BEFORE you do a “final submit” to me.
  • Once you have received your report, please review it. This report will show you grammatical, punctuation, and spelling errors that can easily be fixed. Take the extra few minutes to review instead of getting counted off for these mistakes.
  • Review your similarities. Did you forget to cite something? Did you not paraphrase well enough? Is your paper made up of someone else’s thoughts more than your own?
  • Visit the Writing Center in the Student Success Center, under the Resources tab in LoudCloud for tips on improving your paper and SI score.

Late Policy

  • The university’s policy on late assignments is 10% penalty PER DAY LATE. This also applies to late DQ replies.
  • Please communicate with me if you anticipate having to submit an assignment late. I am happy to be flexible, with advance notice. We may be able to work out an extension based on extenuating circumstances.
  • If you do not communicate with me before submitting an assignment late, the GCU late policy will be in effect.
  • I do not accept assignments that are two or more weeks late unless we have worked out an extension.
  • As per policy, no assignments are accepted after the last day of class. Any assignment submitted after midnight on the last day of class will not be accepted for grading.

Communication

  • Communication is so very important. There are multiple ways to communicate with me:
    • Questions to Instructor Forum: This is a great place to ask course content or assignment questions. If you have a question, there is a good chance one of your peers does as well. This is a public forum for the class.
    • Individual Forum: This is a private forum to ask me questions or send me messages. This will be checked at least once every 24 hours.

Explanation

The goals of explanatory research are to understand the underpinnings of natural phenomena and to explain systematic relationships among them. Explanatory research is often linked to  theories, which are a method of integrating ideas about phenomena and their interrelationships. Whereas descriptive research provides new information and exploratory research provides promising insights, explanatory research attempts to offer understanding of the underlying causes or full nature of a phenomenon. In quantitative research, theories or prior findings are used deductively to generate hypothesized explanations that are then tested. In qualitative studies, researchers search for explanations about how or why a phenomenon exists or what a phenomenon means as a basis for developing a theory that is grounded in rich, in-depth evidence.

Compliance Programs in Healthcare

Your health care organization has had several small compliance incidents in the past two years, and the organization is now motivated to update their compliance program. Your executive leadership team asked you to review two health care compliance programs from similar organizations to determine how they constructed their compliance program and what aspects your organization should adopt.

Select the type of health care organization you want represented in this assignment (e.g., family practice, hospital, urgent care, or nursing home).

Locate two compliance program documents from comparable health care organizations using your Internet search engine.

Read both compliance program documents and examine the similarities and differences between the two.

Create a matrix that compares how both organizations execute the following compliance components:

  • How internal monitoring and auditing is conducted
  • Assignment Compliance Programs in Healthcare

How compliance and practice standards are implemented

  • The designated compliance officer (or person designated to be the contact for compliance matters), who that person reports to, and their relationship to the organization’s governing board
  • How employees are trained and educated to model compliant behaviors
  • How violations or offenses are detected, reported, and corrected
  • How lines of communication with employees is developed
  • How disciplinary standards are enforced

Write a 525- to 700-word executive summary that informs your executive leadership about the matrix you created and offer your opinion as to which best practices the organization should adopt for its own compliance program.

Cite the 2 compliance program documents and any additional references that support your opinion (e.g., trade or industry publications, government or agency websites, scholarly works, or other sources of similar quality).

Format your assignment according to APA guidelines.

Week 1 Discussion

This week, you will start your LinkedIn profile. After you investigate the available resources and review the application, explain why you think this is an important tool for you professionally.

NURS 693B Culminating Experience II

Week 2 Discussion

How is information technology important for the success of an organization? How does it apply to your informaticist role and how will you utilize it? Give specific examples.

NURS 693B Culminating Experience II

Week 3 Discussion

Discuss why your specific research is needed and how will it affect social change, change within an organization, or change within the informatics field in general. Discuss the key people that would be involved or affected if you implement your research and how it will affect them.

NURS 693B Culminating Experience II

Week 4 Discussion

There are several organizations representing informatics professionals. Review the following websites:

https://www.amia.org/

https://www.ania.org/

http://www.himss.org/get-involved/community/nursing-informatics

Discuss whether you feel membership to one or more of these organizations would benefit you in your professional informatics role. Give specific examples of the benefits you would receive. Do you feel there are any negatives to membership? Explain your reasoning for both.

NURS 693B Culminating Experience II

Week 5 Discussion

What was your motivation regarding your research study? What have you found along your research journey that took you in a different direction, surprised you, or confirmed your ideas? Is there anything you would do differently if you were to implement your research?

NURS 693B Culminating Experience II

Week 6 Discussion

Research grant opportunities are specifically available for nursing. Look for grants that might align to your informatics research project. Choose one and describe the steps you would have to take to apply.

NURS 693B Culminating Experience II

Week 7 Discussion

Although you did not implement a full research project, you completed the research proposal with an idea of what your actual results could be. If you ever do complete a full research project, an important step is making sure your research is published/disseminated so that other nurse informatics professionals can benefit from your hard work. There are many ways to disseminate information in nursing as seen in your text, including many different nursing journals. Think about your proposal and in what type of journal it might fit if it were published. Describe how you would disseminate your work and the reasons for your choice.

HCA-827 Topic5 DQ2

Assignment: HCA-827 Topic5 DQ2

Economic downturn commonly means less time in the hospital and greater demand for other services. How might health care leaders best create steady revenue streams amidst changing service requirements created by fluctuating economic conditions? Why? Can steady and sustainable revenue ever be fully achieved? Why or why not? To what extent does the notion of universal health care coverage influence the stability and sustainability of revenue streams for health care organizations? Why?
