Introduction

Suicide remains a major public health crisis in the United States and continues to claim tens of thousands of lives each year despite advances in mental health awareness, treatment, and prevention. According to the Centers for Disease Control and Prevention (CDC, 2023), more than 49,000 people died by suicide in 2022, marking a 3% increase from 2021 and the highest national total on record. This translates to an age-adjusted rate of approximately 14.3 deaths per 100,000 persons, with men dying by suicide nearly four times more often than women. These alarming statistics highlight the persistent and complex interplay between mental illness, socioeconomic stressors, access to care, and cultural attitudes toward mental health.

Understanding suicide trends at the state level provides critical insight into regional disparities and guides the development of targeted prevention strategies. The CDC’s Suicide Mortality by State and Behavioral Risk Factor Surveillance System (BRFSS) data allow health professionals to identify risk patterns related to depression, substance use, unemployment, and limited access to behavioral health services. For instance, states with higher rural populations often report elevated suicide rates due to geographic isolation, stigma surrounding mental health treatment, and reduced availability of psychiatric providers. By examining these data, practitioners can better understand how social determinants of health and public policies affect outcomes within their own states.

Psychiatric-Mental Health Nurse Practitioners (PMHNPs) are uniquely positioned to address these challenges. Their clinical expertise, combined with advocacy and policy engagement, enables them to influence systemic changes that promote mental wellness and suicide prevention. Moreover, PMHNPs can lead efforts to build resilience—both in individuals and within communities—by integrating trauma-informed care, strengthening coping skills, and fostering social connection. This discussion will explore my state’s suicide trends and current prevention initiatives, followed by an analysis of how PMHNPs can engage in policy advocacy and resilience-building to improve mental health outcomes nationwide.

Introduction

Suicide is a profound public health crisis. In 2022, the United States recorded roughly 49,000 deaths by suicide, with an age-adjusted rate of about 14.2 per 100,000 persons. That marked a record high in recent years and underscores the urgency of addressing mental health at both policy and practice levels.  Nationally, the trend over the past decade reflects a roughly 16 % increase in age-adjusted suicide rates (from ~12.3 to ~14.4 per 100,000 between 2011 and 2022).

When I turned to state‐level data for my own state (insert State Name here), I found that in 2022 its suicide rate was X.X per 100,000 (if you have that number). That figure stood out in comparison to national averages. As I explored the BRFSS (Behavioural Risk Factor Surveillance System) prevalence and trends data, I noticed [insert one or two surprising health trends in your state — e.g. rising rates of depression, substance use, chronic disease prevalence, mental health service use, social determinants]. It was surprising to see (for example) how [trend] had increased sharply over the past few years, or how [another trend] lagged behind national benchmarks.

These data point to a clear imperative: we must understand the local context of mental health and suicide to respond effectively. As a Psychiatric-Mental Health Nurse Practitioner (PMHNP), I see the bridges that must be built — integrating clinical care with advocacy, resilience training, and systemic change. In this discussion, I will explore whether my state has initiatives in place to reduce suicide, then consider how PMHNPs can shape health policy and advocacy, and finally discuss how PMHNPs can build resilience in individuals and communities.

ADDITIONAL INSTRUCTIONS FOR THE CLASS

Discussion Questions (DQ)

  • Initial responses to the DQ should address all components of the questions asked, include a minimum of one scholarly source, and be at least 250 words.
  • Successful responses are substantive (i.e., add something new to the discussion, engage others in the discussion, well-developed idea) and include at least one scholarly source.
  • One or two sentence responses, simple statements of agreement or “good post,” and responses that are off-topic will not count as substantive. Substantive responses should be at least 150 words.
  • I encourage you to incorporate the readings from the week (as applicable) into your responses.

Weekly Participation

  • Your initial responses to the mandatory DQ do not count toward participation and are graded separately.
  • In addition to the DQ responses, you must post at least one reply to peers (or me) on three separate days, for a total of three replies.
  • Participation posts do not require a scholarly source/citation (unless you cite someone else’s work).
  • Part of your weekly participation includes viewing the weekly announcement and attesting to watching it in the comments. These announcements are made to ensure you understand everything that is due during the week.

APA Format and Writing Quality

  • Familiarize yourself with APA format and practice using it correctly. It is used for most writing assignments for your degree. Visit the Writing Center in the Student Success Center, under the Resources tab in LoudCloud for APA paper templates, citation examples, tips, etc. Points will be deducted for poor use of APA format or absence of APA format (if required).
  • Cite all sources of information! When in doubt, cite the source. Paraphrasing also requires a citation.
  • I highly recommend using the APA Publication Manual, 6th edition.

Use of Direct Quotes

  • I discourage overutilization of direct quotes in DQs and assignments at the Masters’ level and deduct points accordingly.
  • As Masters’ level students, it is important that you be able to critically analyze and interpret information from journal articles and other resources. Simply restating someone else’s words does not demonstrate an understanding of the content or critical analysis of the content.
  • It is best to paraphrase content and cite your source.

LopesWrite Policy

  • For assignments that need to be submitted to LopesWrite, please be sure you have received your report and Similarity Index (SI) percentage BEFORE you do a “final submit” to me.
  • Once you have received your report, please review it. This report will show you grammatical, punctuation, and spelling errors that can easily be fixed. Take the extra few minutes to review instead of getting counted off for these mistakes.
  • Review your similarities. Did you forget to cite something? Did you not paraphrase well enough? Is your paper made up of someone else’s thoughts more than your own?
  • Visit the Writing Center in the Student Success Center, under the Resources tab in LoudCloud for tips on improving your paper and SI score.

Late Policy

  • The university’s policy on late assignments is 10% penalty PER DAY LATE. This also applies to late DQ replies.
  • Please communicate with me if you anticipate having to submit an assignment late. I am happy to be flexible, with advance notice. We may be able to work out an extension based on extenuating circumstances.
  • If you do not communicate with me before submitting an assignment late, the GCU late policy will be in effect.
  • I do not accept assignments that are two or more weeks late unless we have worked out an extension.
  • As per policy, no assignments are accepted after the last day of class. Any assignment submitted after midnight on the last day of class will not be accepted for grading.

Communication

  • Communication is so very important. There are multiple ways to communicate with me:
    • Questions to Instructor Forum: This is a great place to ask course content or assignment questions. If you have a question, there is a good chance one of your peers does as well. This is a public forum for the class.
    • Individual Forum: This is a private forum to ask me questions or send me messages. This will be checked at least once every 24 hours.

NURS-FPX4025: Applying the PICO(T) Process

Create a 3–5 page submission in which you develop a PICO(T) question for the diagnosis you worked with in the first two assessments and analyze the evidence you locate to answer the question.

Introduction

PICO(T) is an acronym that helps researchers and practitioners define aspects of a potential study or investigation.

It stands for:

  • P – Patient/population/problem.
  • I – Intervention.
  • C – Comparison (of potential interventions, typically).
  • O – Outcomes.
  • T – Time frame (if time frame is relevant).

The end goal of applying PICO(T) is to develop a question that can help guide the search for evidence (Boswell & Cannon, 2015). From this perspective, a PICO(T) question can be a valuable starting point for nurses who are beginning to apply an evidence-based model. By taking the time to precisely define the areas in which the nurse will be looking for evidence, searches become more efficient and effective. Essentially, by precisely defining the types of evidence within specific areas, the nurse will be more likely to discover relevant and useful evidence during their search. When applying the PICO(T) approach, the nurse can isolate the interventions of interest and compare to other existing interventions for the evidenced impact on the outcome of the concern.

Reference

Boswell, C., & Cannon, S. (2015). Introduction to nursing research. Jones & Bartlett Learning.

Professional Context

As a baccalaureate-prepared nurse, you will be responsible for locating and identifying credible and scholarly resources to incorporate the best available evidence for the purposes of enhancing clinical reasoning and judgement skills. When reliable and relevant evidence-based findings are utilized, patients, healthcare systems, and nursing practice outcomes are positively impacted.

PICO(T) is a framework that can help you structure your definition of the issue, potential approach that you are going to use, and your predictions related to the issue. Word choice is important in the PICO(T) process because different word choices for similar concepts will lead you toward different existing evidence and research studies that would help inform the development of your initial question. When writing a PICO(T)-formatted research question, you want to focus on the impact of the intervention and the comparison on the outcome you desire.

Scenario

For this assessment, please use the diagnosis you worked with in the first two assessments.

Instructions

After reviewing the materials you created to research a specific diagnosis in the first two assessments, apply the PICO(T) process to develop a research question and research it.

Your initial goal is to define the population, intervention, comparison, and outcome. In some cases, a time frame is relevant and you should include that as well, when writing a question you can research related to your issue of interest. After you define your question, research it, and organize your initial findings, select the two sources of evidence that seem the most relevant to your question and analyze them in more depth. Specifically, interpret each source’s specific findings and best practices related to your chosen diagnosis, and explain how the evidence would help you plan and make decisions related to your question.

If you need some structure to organize your initial thoughts and research, the PICO(T) Question and Library Search template (accessible from the Create PICO(T) Questions page) might be helpful.

In your submission, make sure you address the following:

  • Review the PICO(T) process.
  • Explain your diagnosis from Assessment 1 in terms of outcomes, risks, and complications. In your explanation, be sure to include the risks and complications for vulnerable populations or those affected by healthcare disparities and the impact of those disparities.
  • Consider your diagnosis from Assessment 1. Develop a research question using the PICO(T) process to address an issue of your choosing related to your diagnosis.
  • Properly format your PICO(T) question to guide your search of the literature.
  • Describe your search of the literature, noting search engines, key words, and credibility factors.
  • Locate evidence that explores your intervention and comparison of your PICO(T) question with an emphasis on your specific outcome (at least three diagnosis-issue-specific sources of evidence (scholarly articles)).
  • Explain the content of each of your scholarly articles.
  • Explain the credibility and relevance of each of the articles you chose to your diagnosis issue.
  • Analyze the evidence. Determine the answer to your PICO(T) question based on your analysis of the evidence.

Additional Requirements

Your assessment should also meet the following requirements:

  • Length of submission: Create a 3–5-page submission focused on defining a research question and interpreting evidence relevant to answering it.
  • Number of references: Cite a minimum of four sources of scholarly or professional evidence that support your findings and considerations. Resources should be no more than 5 years old.
  • APA formatting: Format references and citations according to the current APA style.

Competencies Measured

By successfully completing this assessment, you will demonstrate your proficiency in the following course competencies and scoring guide criteria:

  • Competency 1: Interpret findings from scholarly quantitative, qualitative, and outcomes research articles and studies.
    • Describe a search of the literature noting search engines, key words, and credibility factors.
  • Competency 2: Analyze the relevance and potential effectiveness of evidence when making a decision.
    • Explain a diagnosis in terms of outcomes, risks, and complications, including the risks and complications for vulnerable populations or those affected by healthcare disparities, and the impact of those disparities.
    • Explain the content of at least three sources of evidence, including the credibility and relevance of the articles to a specific diagnosis issue.
  • Competency 4: Plan care based on the best available evidence.

Introduction

Diversity, equity, and inclusion (DEI) are foundational principles that continue to transform the landscape of modern healthcare. As the patient population becomes increasingly diverse, healthcare professionals must cultivate environments that recognize and respect differences in culture, race, ethnicity, gender, age, and belief systems. DEI initiatives promote fair treatment, equal access to care, and an inclusive workplace culture—key factors that directly influence patient outcomes and satisfaction. Understanding DEI and its ethical and practical implications ensures that nurses and healthcare leaders deliver care that is not only competent but also compassionate and just.

This assessment provides an opportunity to analyze the evolution of DEI within healthcare, explore the impact of unconscious bias and microaggressions, and identify effective strategies for promoting fairness and inclusivity in professional practice. By reflecting on both historical progress and ongoing challenges, this paper aims to highlight the importance of cultural humility and self-awareness in reducing disparities and improving communication across diverse populations.

Furthermore, the discussion emphasizes the role of continuous education and organizational initiatives in addressing unconscious bias—an often invisible barrier to equitable care. Through intentional efforts such as bias training, diverse leadership, and culturally responsive care models, healthcare systems can foster trust and build stronger patient–provider relationships. Ultimately, advancing DEI principles in healthcare is not only a moral imperative but also a clinical necessity for achieving equitable health outcomes and ensuring that every patient feels valued, heard, and respected.

Introduction

Ethical decision-making is central to professional nursing practice and influences every aspect of patient care. Nurses are expected to uphold moral and professional standards while making decisions that balance patient needs, organizational policies, and societal expectations. This assessment applies the four core ethical principles—autonomy, beneficence, nonmaleficence, and justice—to a healthcare issue previously explored in the Applying Research Skills assessment. Through this application, the aim is to analyze how ethical reasoning guides nursing actions and ensures that care remains fair, compassionate, and patient-centered.

Healthcare professionals encounter complex dilemmas that require critical thinking, moral judgment, and an understanding of both individual and collective responsibilities. Applying ethical principles provides a structured framework for addressing these challenges and maintaining integrity in clinical practice. Furthermore, recognizing the influence of bias—whether personal, cultural, or systemic—is vital in ensuring that care decisions remain equitable and objective. Addressing bias enhances fairness and promotes ethical alignment across all aspects of patient care.

This assessment also integrates one of the Four Spheres of Care to demonstrate how ethics connects to specific domains of nursing practice, such as wellness and disease prevention, chronic disease management, restorative care, or palliative care. Understanding ethics within these spheres not only supports professional accountability but also reinforces the nurse’s role as an advocate for dignity, justice, and patient autonomy.

Ultimately, this presentation emphasizes how the consistent application of ethical principles ensures responsible, evidence-based, and human-centered care in today’s complex healthcare environment.
