The targeted patient population is an issue that should be addressed. For instance, aiming at the expensive multimorbid patients gives the highest prospect for escalated effectiveness. Care integration demands great consideration of the necessities of the patients and threat prominence (Ferrer, 2015). Additionally, authorizing admission to such data by various clinicians for the maintenance of the patients’ confidentiality.

Economic success is an issue that should be considered in the healthcare integration. Finances are vital in the implementation of the process. This is simply because to achieve the set objectives, vision and strategy capital must be available. Alignment with the medical staff should also be put into consideration for the increase of patient acceptance and success of the program. Physicians are needed for the assessment of specific needs of the patients and recommendation of appropriate interventions leading to achievement of set objectives. In so doing the evidence-based benefits of the program will be achieved. For example, appropriate therapies for different patients will clearly be ordered.

In conclusion, shared condition for the provision of reliable and coordinative care should be considered in association of basic and communal amenities so that the care basically performs as the background of executing care integration.
