1. A learning psychologist is interested in comparing the success of 2 different mnemonics (memorization methods) on performance in a memory task. He assigns students to two groups, one which learns and uses a language-based rhyming memory technique, and one which learns and uses a visual “method of loci” spatial memory technique. He then administers a memory task to each group of students. In the case of Maria, primary hypothyroidism is a medical condition where the thyroid gland does not produce enough hormones. Treatment for this condition includes hormone replacement therapy with synthetic thyroid hormones. The goal of this treatment should be to restore normal thyroid hormone levels in the body, reduce signs and symptoms, and minimize the risk of long-term complications (Bilous et al., 2021). The doctor should aim to monitor her progress to assess how well the treatment is working. This can be done by regularly checking her blood work for any changes in hormone levels. She may need to adjust her dosage to achieve the desired results if needed.

The doctor should also assess Maria regularly to ensure the treatment is effective and to spot any side effects. While treating Maria, it is important to provide her with emotional support and education. She should be aware of the signs and symptoms of hypothyroidism and be able to recognize any changes in her health (Bilous et al., 2021). With the right support, Maria can make lifestyle changes that can be beneficial in optimizing her treatment outcomes.

What drug therapy would you prescribe? Why?

In the case of Maria, a 29-year-old woman with a seven-month history of heavy, irregular menses, weight gain, constipation, and decreased energy, I would recommend levothyroxine, a synthetic form of the hormone thyroxine, to treat her primary hypothyroidism. This hormone helps regulate metabolism, energy, and body temperature and is the most commonly prescribed drug for patients with this condition. The dosage, frequency, and duration of the drug therapy depend on the degree of the patient’s hypothyroidism and their individual needs; however, I would typically prescribe a daily dosage of 25-50 mcg. This dosage can then be increased gradually until the desired level is achieved. Depending on the patient’s individual needs, the frequency and duration of the drug therapy may need to be adjusted. Always monitor the patient’s TSH and T4 levels regularly to ensure the dosage is effective (Effraimidis et al., 2021). In summary, to treat Maria’s primary hypothyroidism, I would prescribe levothyroxine, with a starting daily dosage of 25-50 mcg, and adjust the dosage, frequency, and duration as required. Regular monitoring is also vital for ensuring the effectiveness of the medication.

What are the parameters for monitoring the success of the therapy?

Once a diagnosis of primary hypothyroidism is made, it is important to monitor the patient’s progress with therapy. The most reliable way to monitor the success of therapy is to regularly measure the patient’s Free T4, Thyroid Stimulating Hormone (TSH) level, and thyroid peroxidase (anti-TPO) antibodies. Additionally, indicators such as symptoms, body weight, blood pressure, physical examination, and electrolyte levels should be monitored to help determine if the patient is responding to treatment. In Maria’s case, it is important to monitor her Free T4, TSH, and anti-TPO antibody levels to ensure they return to normal levels (Fitzgerald & Falhammar, 2022). Her progress should also be monitored via physical examination, weight, and blood pressure. The patient should also be evaluated for other thyroid-related conditions, such as thyroid nodules or Graves’ Disease, as these can significantly impact the success of treatment. Furthermore, any changes in symptoms, such as an increase in energy levels or a decrease in menstrual problems, should also be monitored as indicators of success with therapy.

Discuss specific patient education based on the prescribed therapy.

Given Maria’s diagnosis of primary hypothyroidism, it is important for her to be provided with adequate patient education regarding the prescribed levothyroxine therapy. Patient education typically involves providing Maria with information about her condition and medications. As such, it is important to stress the importance of taking the prescribed levothyroxine to achieve and maintain optimal health. Additionally, Maria should be informed of the potential side effects of the medication, as well as any signs and symptoms that could indicate that a change in dosage or additional medical attention is necessary (Jonklaas et al., 2021). Additionally, patient education should include an explanation of the importance of taking the medication regularly and at the same time of day each day to ensure optimal absorption of the medication. Additionally, Maria should be encouraged to avoid taking food or supplements containing iron or calcium within four hours of taking the levothyroxine, as this can interfere with the absorption of the medication. Furthermore, it is important to discuss follow-up plans with Maria to assess the medication’s effectiveness and make any necessary adjustments or changes.

List one or two adverse reactions for the selected agent that would cause you to change therapy.

In the case of Maria, who has primary hypothyroidism, it is essential to monitor the treatment with levothyroxine carefully. In particular, Maria should focus on changes to her diet, exercise habits, and stress levels. First and foremost, it is important to ensure that Maria has a balanced and healthy diet (Mahdavi et al., 2021). Consuming foods high in nutrients, vitamins, and minerals can help her maintain a healthy weight, boost her immunity, and improve overall energy levels, which the condition may hamper. Additionally, it is important to be mindful of Maria’s current iron and calcium supplement intake, opting for food sources of these important minerals instead.

In terms of exercise, Maria should talk to her healthcare provider to establish a tailored exercise plan to help her maintain a healthy weight and increase her energy levels. Since primary hypothyroidism can cause tiredness, fatigue, and malaise, the exercise plan should be reasonable. Rather, it should consist of low-impact aerobic exercises, such as swimming and walking, to boost her energy without taking too much from her (Mahdavi et al., 2021). Finally, Maria should take steps to reduce her stress levels. This often requires lifestyle changes, such as simplifying one’s life and focusing on activities that bring joy and relaxation. Mindfulness practices such as yoga, tai chi, and meditation can also help to reduce stress. Additionally, Maria needs to get proper rest to help her body cope with the condition’s symptoms.

Describe one or two drug–or drug–food interactions for the selected agent.

Patients taking levothyroxine are advised to avoid eating soy products, fiber-containing foods, and calcium supplements within four hours of taking the medication, as these could interfere with the drug’s absorption. Maria is a 29-year-old woman with a seven-month history of heavy, irregular menses, a 5-lb weight gain, constipation, and decreased energy. Her past history is unremarkable. She takes no prescription medications but uses iron and calcium supplements. She has a family history of thyroid disease. On examination, her weight is 152 lbs, her heart rate is 64 bpm, and her blood pressure is 138/86. Her thyroid gland is mildly enlarged, without nodularity. She has trace edema in her lower extremities, and her reflexes are slow.

 

Laboratory studies are as follows:

 

TSH is 15.3 mIU/mL (elevated), free T4 is 0.3 mIU/mL (decreased), and total cholesterol is 276 mg/mL.

Clinical Experience and Patient Assessment in a Primary Care Setting

This week’s experience working as a student nurse practitioner in a primary care clinic allowed me to integrate better the knowledge I have learned in school and acquaint myself with my student nursing skills. During auscultation, I detected wheezes and the patient’s elevated respiratory rate; the chart revealed that she has seasonal allergies and spends most of her time in her chest. These data suggest that the clinician needs to meet her diagnostic goals and help her maintain optimal lung health to give an effective treatment plan. Breathlessness, wheezing, and coughing were all part of the patient’s bronchial asthma symptom complex.

The patient may be diagnosed with asthma, COPD, or allergic rhinitis, among other diagnoses that frequently confound medical professionals. Based on the patient’s medical history, there were indicators that the associated allergies and respiratory issues could potentially cause an asthma attack. Therefore, in this case, the asthma attack was actual. When these symptoms are present, especially in frequent smokers, another condition may be the primary cause of chronic cough and dyspnea. The concurrent presentation of seasonal diseases, which can be linked to the history of allergies, is one of the best indicators for diagnosing allergic rhinitis (Jansen et al., 2021). We carefully looked into the matters that were crucial to this investigation. After that, we monitored and evaluated the patient’s tidal volume until it died.

A patient is prescribed a long-term inhaler, which works on the airways to prevent the inflammation from worsening and producing negative symptoms. Furthermore, a quick-acting bronchodilator can be taken immediately in an emergency to treat a worsening disease. I also demonstrated to them how to use the examples and recommendations for the allergen avoidance strategy. In order to do this, adding nasal corticosteroid spray to the medication will be the best option. In order to lessen exposure to allergens, this research discusses occupational health promotion methods such as using inhalers, quitting smoking, and changing the office environment (Keogan et al., 2021). This one-day clinical encounter indicates that the supervisor’s use of an evidence-based medicine concept-based research and analytical evaluation technique is a viable means of diagnosing and treating respiratory disorders in a respiratory specialist. In addition to my proficiency with technology, typical patient plans or directions, and analytics, I want to demonstrate my care. I am going to make humility, empathy, and compassion manifest. Improving patient care and, as a result, my patients’ clinical outcomes are some of the abilities I would like to include in my practice.

I strongly encourage that you do not wait until the very last minute to complete your assignments. Your assignments in weeks 4 and 5 require early planning as you would need to present a teaching plan and interview a community health provider. I advise you look at the requirements for these assignments at the beginning of the course and plan accordingly. I have posted the YouTube link that explains all the class assignments in detail. It is required that you watch this 32-minute video as the assignments from week 3 through 5 require that you follow the instructions to the letter to succeed. Failure to complete these assignments according to instructions might lead to a zero.
