Assignment 3 Social Determinants of Health Photo Reflection Assignment Overview and Expected Outcomes This assignment is designed to assess the understanding of social determinants of health and disease. Upon completing the assignment students are expected to: 1. Define the leading social determinants of health. 2. Illustrate the intersection of social determinants, vulnerable population, culture, and pharmacy. Guidelines for Completing Assignment 1. This is an individual assignment. Each student must select TWO social determinants of health and take TWO photographs that illustrate the intersection of the social determinants, vulnerable population, culture and pharmacy. 2. The TWO photographs must be annotated with  A title  Social determinant of health represented  Two paragraphs indicating how the selected determinant is linked to health and disease; and the role that pharmacy could play in addressing the determinant from a cultural perspective. Guidelines for Submitting Assignment 1. The assignment should be submitted as a Word document through Canvas under Assignment 3- Social Determinants of Health Photo Reflection no later than OCT 22 @ 11:59 p.m. Sources used to support your ideas the must be cited using AMA format on a work-cited page at the end of the document. 3. Assignments that are not uploaded to the Canvas will not be accepted. DO NOT email assignments directly to the course faculty or coordinators. Assignments must be submitted by the due date. Ten percentage points will be deducted from the final grade for each day the assignment is late after the submission deadline.
