For this reason, a person-centered approach will be ideal. It will not only ensure that the caregiver focuses o the essential aspects of the required care. Patient-centered care is a fundamental approach that accentuates eight key characteristics, which include the following. This approach fosters dignity, compassion, and respect when treating any patient (Hashim 2017). It is common for patients to lose their independence when they enter care. This way, they are likely to lose their dignity. Showing respect to their wishes will often give them a sense of confidence in their pride. For this reason, it is imperative to understand that the patients have their thoughts, feelings, values, and opinions. Invalidating such aspects could easily send them to further emotional or even physical breakdown. This form of care should always provide coordinated care, treatment, and support (Tzelepis, Sanson-Fisher, Zucca & Fradgley 2015). Coordination with other key players, including social workers and nurses, will be helpful in this pursuit. They will help in rebuilding and even understanding the personal needs and wants of the patients. This way, it will be easier to customize a care plan for the given patient. Respect for the patient will also mean that the patient’s rights will be observed, and unethical issues will not occur (Guido, 2002). Information sharing: This type of care requires open communication between health care professionals and patients. Information such as the type of disease, diagnosis, and medication plan is crucial because it prepares patients and their families psychologically towards particular treatment. Another characteristic is the involvement of family and friends. This is important because involving these people will provide moral support to the patient, which is vital for the patent’s recovery process. The patient will also feel loved when these people are around him or her. Steps to provide patient education including cultural consideration Patient education for aging patients requires some procedures and requirements. I will slow the pace of presentation as well as using repetition to ensure that they hear and understand because aging people experience difficulty in hearing (Gomez, 2016). Another step involves providing the patient with a small amount of information so that he can have enough time to consume and understand. Additionally, I will use examples from everyday experience when illustrating general information to ensure that my patient understands what I would be explaining. I will also use relevant terminologies and incorporate the group’s cultural considerations in my teaching to ensure that I don’t go against the culture of the patient. How you will evaluate the effectiveness of your education There are various ways that I can use to assess the effectiveness of my education. First, if the patient responds positively to my education. For example, asking questions or clarification on what I have taught. Another way is when the patient is satisfied with my teaching. This can be realized from the patient’s comment, such as “thank you I have understood.” In addition to this, I will identify the effectiveness when the patient follows the prescription. Additionally, I can evaluate the effectiveness of education by pointing out barriers that prevent learning. Being able to identify barriers means that you are keen and that you follow what you are teaching (Masters, 2016).
