What I Did Right

By creating a comprehensive health history, I have demonstrated proficiency in all aspects of the endeavor. I established a firm rapport with the patient early on, which is essential for eliciting candid responses. By employing nonverbal indicators such as a friendly smile, prolonged eye contact, and attentive listening, I created a safe environment for the patient. The patient could communicate their medical concerns and provide vital medical information.

I added open-ended questions to my arsenal of techniques during the discussion of the patient’s medical history to elicit comprehensive responses from the patient. It has been determined that asking follow-up inquiries to elicit additional information and clear any misunderstandings is an effective strategy(Atsma et al., 2020). This method facilitated a thorough evaluation of the patient’s physical and mental health, providing a complete picture of their current state of health. I could address the patient’s current concerns and any potential underlying issues by employing this technique. Another way I demonstrated my cultural competence was by attempting to comprehend the patient’s cultural upbringing and values. I knew the importance of adapting my strategy to their country’s beliefs and preferences to facilitate straightforward communication and mutual understanding between all parties(Atsma et al., 2020).

Areas for improvement

Medical history compiling is an area where I can still improve despite my accomplishments. Time management ability stands out as one of the most essential characteristics. One aspect of the patient’s health history that I occasionally spent excessive time on was the patient’s medical history. As a result, I had less time to discuss psychosocial and lifestyle concerns with the patient. This may make it difficult to obtain a more accurate picture of the patient’s health (Kadhim et al., 2020).

One more area in which I need to improve is my ability to ask more specific and insightful questions. Even though I did an excellent job of employing open-ended questions, I could enhance my performance by creating more focused queries that extract specific information about the patient’s symptoms, previous medical history, and family history. If I were to do so, I would be able to collect more pertinent and comprehensive data, which would assist me with both the diagnosis and the treatment planning (Roodbari et al., 2021).

Last but not least, I need to work on improving my documentation skills. It is crucial to have precise and complete documentation to ensure that patients receive consistent care and that the healthcare team members can communicate effectively with one another. During the health history interview, I need to develop my note-taking techniques to ensure that all relevant information is captured accurately and thoroughly.

Strategies for Improving

I intend to put numerous plans into action to address the areas that need improvement:

  1. Time management: I will develop a methodical framework for asking people about their medical histories. Using this framework, I will set aside specific time slots for the various parts of history, ensuring that I have enough time to cover every important detail, from the medical background to the psychological components(Doty et al., 2021).
  2. Questioning Techniques: Using fictitious health history interviews as practice, I will work on developing more focused queries. In order to improve my asking strategies and ensure I ask appropriate questions that result in a thorough assessment, I will also consult experienced healthcare professionals for their comments (Doty et al., 2021).
  3. Documentation: I will spend some time honing my interview note-taking techniques for the health history. This involves writing down patient information clearly and succinctly while still being thorough enough for other healthcare professionals to grasp(Doty et al., 2021).

In conclusion, taking a health history is a crucial skill in the medical field, and my evaluation of the assignment has highlighted my strengths and my need for growth. I want to become a more capable healthcare professional who performs complete and efficient health histories, contributing to better patient treatment and outcomes. I will achieve this by emphasizing time management, honing my questioning techniques, and improving my documentation procedures.

After reading these articles, create a 10-15 slide presentation in the format shown below.

Slides 1 and 2 depict the hormonal signal cascade described in Randall’s (2011) article’s section titled “Neurochemistry of Stress.” To represent the regions that release specific hormones in the hormone cascade that results in cortisol release, use simple ovals or circles. Textboxes can be used to label the hormones, regions, and organs involved (both options are available in the PowerPoint program’s “Insert” tab).

Label and diagram the kidney and adrenal gland on slides 3 and 4. Include the following items:

Cortex and medulla of the adrenal gland; regions associated with hormone production and release; hormones released in each region
Kidney cortex and medulla; water absorption and filtering region; urine collection region

Diagram and label the HPA axis on slide 5. Include the hormones produced by each component as well as their impact on the target organ (s).

Slides 8-10: Conduct additional PTSD research. Draw a diagram of the brain regions thought to be involved in PTSD symptoms. Explain the connection between PTSD and depression, and include relevant brain regions in your labeled regions.

Slides 11-13: Summarize the various findings in the studies described in the article “The Relationship between Traumatic Stress, PTSD, and Cortisol” in bulleted points. Include the correlations discovered between cortisol levels and PTSD patients. Some explanations from the article for the variability in cortisol levels in these individuals are included in the notes. Explain the limitations of the studies mentioned at the end of the article by the author.

In a competitive, monetized medical system, a clash of principles is inevitable; however, amid the tumult in the medical marketplace, the underlying tenets of emergency medical ethics remain inviolate. Leadership Theories in Practice

A walk through the Business section of any bookstore or a quick Internet search on the topic will reveal a seemingly endless supply of writings on leadership. Formal research literature is also teeming with volumes on the subject.

However, your own observation and experiences may suggest these theories are not always so easily found in practice. Not that the potential isn’t there; current evidence suggests that leadership factors such as emotional intelligence and transformational leadership behaviors, for example, can be highly effective for leading nurses and organizations.

Yet, how well are these theories put to practice? In this Discussion, you will examine formal leadership theories. You will compare these theories to behaviors you have observed firsthand and discuss their effectiveness in impacting your organization.

To Prepare:

Review the Resources and examine the leadership theories and behaviors introduced.

Identify two to three scholarly resources, in addition to this Module’s readings, that evaluate the impact of leadership behaviors in creating healthy work environments.

Reflect on the leadership behaviors presented in the three resources that you selected for review.

Discussion Questions (DQ)

Initial responses to the DQ should address all components of the questions asked, include a minimum of one scholarly source, and be at least 250 words.

One or two sentence responses, simple statements of agreement or “good post,” and responses that are off-topic will not count as substantive. Substantive responses should be at least 150 words.

Weekly Participation

Your initial responses to the mandatory DQ do not count toward participation and are graded separately.

Familiarize yourself with APA format and practice using it correctly. It is used for most writing assignments for your degree. according to NSG 601 Module 7 Discussion, a barrier to change that is noted in the literature is staff resistance. This is typical because of the belief in increased workloads or disruption of familiar workflow patterns (Laker et al., 2019). Sufficient time should be allocated for training so staff can receive adequate training and education with respect to the unique needs of patients. Another barrier to change is culture change. Organizational culture change or adjustment should be addressed by leadership that is completely transparent with data, clear and positive messages to staff, and providing constant reinforcement to staff about the importance of following new processes (Laker et al., 2019).

Rationale: The process of triage in the emergency department (ED) allows prioritization of treatment based on presentation and acuity as stated in NSG 601 Module 7 Discussion. When patients board in the ED, these processes get impeded, and it causes a bottleneck effect which ultimately causes a cascade of other issues. This tagline gives a simple perspective to something that most adults have experienced or at least witnessed. The strategy is to promote a positive streamline in the system by providing evaluation, stabilization, and treatment for all patients.

Consider a creative communication tool to demonstrate your tagline to the group.  Youtube, a short video, song, demonstration, or graphic would be ideal!

What a creative tagline and association to “jammed traffic” it really makes me think about the effects of boarding in the ED “downstream”.  I do not know anything about patient flow in the ED, however I do know the effects of closing lanes on traffic and the problems that can arise. When I was a floor RN I remember having to take ED patients even when our floor was in absolute chaos; if the patient was stable he went to the floor. I can not imagine having patients, especially psychiatric, boarding in the ED. Does this impact the ED nurse to patient ratio? The ED is not designed to meet needs associated with the mentally ill, these visit occupy 42% increased provider time (Nordstrom et al., 2019). I would imagine that if any interventions help alleviate this time constraining factor, there would be 100% buy in.

NSG 601 Module 7 Discussion states that, you make a number of good points. I worked in the ED for 2 years and it is a completely different world than the floors or ICU. The organization I worked at used an acuity system, but the problems arose when this is not used correctly. At times the triage nurse would not get the complete story. This leads to acuity changes. For instance, if a patient comes in with N/V, but does not indicate they hit their head, this initiates two different treatment plans.

Boarding in the ED is a nightmare. Do you feel it has increased due to COVID or is there a pattern throughout the year/seasons. I am currently working in ICU and we have had to take floor patients to get them out of the ED and board in ICU. This creates further problems as it uses critical care resources that are now not available to the patients and hospital.

Both are difficult to change without good leadership from the organization. In your tagline, you discuss the process of streamlining the system in the emergency department through triage. Working in the emergency department myself I fully understand overcrowding and boarding of patients as it’s a real crisis. Without control in the front (triage) then it makes it extremely difficult to manage care appropriately for patients waiting to be seen in the ED. Currently, at my assignment, the triage area has a physician that sees each patient, assesses and evaluates them, while providing treatment or orders to assist in spreading up the care of the patients. This decreases delays in care and increases patient morale all while maintaining quality patient care. Having tools and guidelines in triage is essential to perform appropriate assessments and pair the correct acuity level based on current symptoms to each patient. In one article I found they used a tool called the AIFELL score. according to NSG 601 Module 7 Discussion, this triage score considered symptoms, laboratory studies, and radiological images to identify appropriate admissions of people with COVID-19 who are critical patients (Levenfus et al., 2020). The tool allowed for a more streamlined approach as the pandemic made it difficult to know who is most appropriate for admission.

1) The EHR has made a major difference on safety, there is no doubt. How do you think informatics play a role in developing and managing protocols? How do these different methods of treatment effect patient safety?
2) I agree that EMR can help improve the quality of care given to patients by having all of the patients’ information readily available to all treatment providers. Wakefield (2008) states, “Another report recommendation called for professional organizations to firmly commit to an agenda focused on patient safety, with specific efforts targeted toward health professions education” (p. 5). I believe continued education for everyone involved in health care is essential to provide the best care possible. What are some “current rules” and “new rules” mentioned by Wakefield (2008) have you witnessed in your practice/organization? It is interesting to see how many organizations are working towards moving to the “new rules” and how many are still practicing “current rules.” Great post!

Due Date: You are to identify a total of five research databases (or collections of research-based evidence) that are relevant to a significant clinical nursing issue that is important to you. Submit the descriptions using the form provided in Doc Sharing to the Week 3 Dropbox by 11:59 p.m. Sunday (MT).

The assignment is worth a total of 175 points. Please see the grading rubric for details.

Requirements
1.  Choose a topic of interest to you that is a significant clinical nursing issue. Please note that the databases you identify could be useful sources of information for your Capstone project in NR451, so choose your topic thoughtfully.

2.  Download the NR439_Research_Database_Form from Doc Sharing and type information about each database directly onto the form. Your paper does NOT need to follow APA formatting; however, you are expected to use correct grammar, spelling, syntax, and write in complete sentences.

3.  Save the file by clicking Save as and adding your last name, e.g., NR439_Research Database_Smith.docx.

4.  Describe your topic of interest. This is worth 15 points.

5.  Locate a total of five research databases  related to your topic. Each research database description is worth 30 points. You may search for these in various locations, e.g., the Internet or the CCN Library.  You may NOT choose the databases that are already familiar to you – MEDLINE, PUBMED (which also indexes MEDLINE), GOOGLE SCHOLAR, and CINAHL. Instead, you are expected to expand your knowledge of evidence-based sources. Places to help you begin include your textbook, which lists databases and websites that report research. The Chamberlain online library (http://library.chamberlain.edu)  gives you access to several databases and provide tutorials for searching. Government sites such as those at the National Institutes of Health offer collections of research on a variety of subjects. The key to choosing the databases is that each contains research-based evidence that also pertains to your topic of interest. Again you are looking for new databases to use in the future and not articles related to your nursing clinical issue.

6.  Review each database to gather information to create a description for the assignment. Each description must:

a.  identify the title of the research database;

b.  describe the location of the research database in a way that a reader could find it. This should be the URL. This URL should take you to the home page of the database.

c.  name owner or publisher of the source;

d.  describe the research database. This must be in your own words and not copied and pasted from the original source. Include the purpose of the database and the subject matter it covers. This may be two to three sentences; and

e.  explain how the research found in the database relates to your topic of interest. This may be an two or three sentences.

7.  Submit to the Research Database basket in the Dropbox by 11:59 p.m. MT Sunday at the end of Week 3. Please post question about this paper in the Q & A Forum.

  • Participation posts do not require a scholarly source/citation (unless you cite someone else’s work).
  • Part of your weekly participation includes viewing the weekly announcement and attesting to watching it in the comments. These announcements are made to ensure you understand everything that is due during the week.

APA Format and Writing Quality

  • Familiarize yourself with APA format and practice using it correctly. It is used for most writing assignments for your degree. Visit the Writing Center in the Student Success Center, under the Resources tab in LoudCloud for APA paper templates, citation examples, tips, etc. Points will be deducted for poor use of APA format or absence of APA format (if required).
  • Cite all sources of information! When in doubt, cite the source. Paraphrasing also requires a citation.
  • I highly recommend using the APA Publication Manual, 6th edition.

Use of Direct Quotes

  • I discourage overutilization of direct quotes in DQs and assignments at the Masters’ level and deduct points accordingly.
  • As Masters’ level students, it is important that you be able to critically analyze and interpret information from journal articles and other resources. Simply restating someone else’s words does not demonstrate an understanding of the content or critical analysis of the content.
  • It is best to paraphrase content and cite your source.

LopesWrite Policy

  • For assignments that need to be submitted to LopesWrite, please be sure you have received your report and Similarity Index (SI) percentage BEFORE you do a “final submit” to me.
  • Once you have received your report, please review it. This report will show you grammatical, punctuation, and spelling errors that can easily be fixed. Take the extra few minutes to review instead of getting counted off for these mistakes.
  • Review your similarities. Did you forget to cite something? Did you not paraphrase well enough? Is your paper made up of someone else’s thoughts more than your own?
  • Visit the Writing Center in the Student Success Center, under the Resources tab in LoudCloud for tips on improving your paper and SI score.

Late Policy

  • The university’s policy on late assignments is 10% penalty PER DAY LATE. This also applies to late DQ replies.
  • Please communicate with me if you anticipate having to submit an assignment late. I am happy to be flexible, with advance notice. We may be able to work out an extension based on extenuating circumstances.
  • If you do not communicate with me before submitting an assignment late, the GCU late policy will be in effect.
  • I do not accept assignments that are two or more weeks late unless we have worked out an extension.
  • As per policy, no assignments are accepted after the last day of class. Any assignment submitted after midnight on the last day of class will not be accepted for grading.

Communication

  • Communication is so very important. There are multiple ways to communicate with me:
    • Questions to Instructor Forum: This is a great place to ask course content or assignment questions. If you have a question, there is a good chance one of your peers does as well. This is a public forum for the class.
    • Individual Forum: This is a private forum to ask me questions or send me messages. This will be checked at least once every 24 hours.
