Theoretical and Research Base: Creative Learning Environments

The work of Urie Bronfenbrenner (2004), Maria Montessori (1909, 1964), Loris Malaguzzi (1995), and Lev Vygotsky (1967, 1978), among others, provide important insights into creative environments that engage all children. Following is a brief statement of each of these theorists’ assumptions about the influence of the environment on children’s creativity and how their theories might look in early childhood classrooms.

Bronfenbrenner

From Bronfenbrenner we learn about the important interactions of many environments, such as the family, school, neighborhood, peers, and media that are all connected and influence not only one another but also the developing child. His theory provides one way to view the effects of the social contexts of children’s lives on the child in the classroom.

An early childhood classroom influenced by Bronfenbrenner’s theory would include:

· Strong connections between home and school by listening to what families have to say about their children and their home interests so that both teachers and children can learn about every child’s community and culture.

· Families that are involved in children’s learning activities that you send home.

· Family members that are involved in a variety of roles in the classroom.

· Strong relationships with the community.

Montessori

From Montessori we learn that children need a carefully prepared, well-organized environment with authentic, homelike materials to reflect order and calm. The environment contains aesthetically pleasing and sensory-rich materials, child-sized furnishings, and self-correcting materials to be used in a specific way. Teachers carefully structure the environment for the children to complete tasks and develop at their own pace.

This girl is building a tower using Montessori cylinders in a prepared environment

A classroom environment influenced by Montessori would have:

· An aesthetically pleasing classroom with a wide selection of sensory materials and experiences for self-expression.

· Low shelves with materials that children can access easily and return materials to their original place.

· Large, open floor spaces.

· Considerable freedom for children to choose activities that have been prepared by the teacher.

· Teachers who respect children, guide their use of materials, and offer help if asked.

Malaguzzi and Reggio Emilia Schools

Malaguzzi calls the classroom environment the child’s “third teacher,” which conveys its powerful impact on children’s thinking and feeling. In Reggio schools, environments are places of beauty that are designed to promote children’s relationships, sense of community, and aesthetics. They are also places that value children’s relationships as a basis of learning. Reggio teachers respect children’s curiosity, ask focused questions, document children’s learning, and display children’s work that reflects their conversations, interests, and experiences.

This video shows key principles of the Reggio Emilia approach to early childhood education. Notice the Reggio environment. How does it impact children’s creative thinking?

Classrooms inspired by Malaguzzi and Reggio Emilia schools would have:

· An aesthetically pleasing environment with lots of light and welcoming entryways.

· Children collaboratively exploring topics of interest to them for long periods of time.

· A variety of open-ended materials and media that stimulate children’s senses and curiosity and encourage investigation, inquiry, and discovery.

· Places for children’s “in progress” projects or products.

· Displays of children’s work that show children and their work are valued and respected.

Vygotsky

Vygotsky theorizes that a hands-on, interactive environment is children’s opportunity to work together. Teachers scaffold children’s thinking and relationships with one another. They guide children in creating themes based on their interests and focus on child-directed play for preschool children and productive activities in the primary grades.

Environments based on Vygotsky’s ideas would have:

· Small-group work that focuses on social interaction and learning from one another.

· Choices of projects for which children can seek help if needed.

· Dramatic play that includes children’s plans of what they want to do to increase the complexity of their play.

· Teachers who serve as partners in learning until children can apply a skill on their own.

Each of these theorists helps us understand the importance of the environment in promoting children’s creative thinking. Now, recall some of your own classrooms in which you were comfortable, felt valued, and looked forward to learning as compared to those in which you were uncomfortable, felt devalued, and felt like learning was a chore. Think about those classrooms as you read about the elements of creative classroom environments.

An aesthetically pleasing environment with lots of light impacts children’s creativity

ADDITIONAL INSTRUCTIONS FOR THE CLASS

Discussion Questions (DQ)

  • Initial responses to the DQ should address all components of the questions asked, include a minimum of one scholarly source, and be at least 250 words.
  • Successful responses are substantive (i.e., add something new to the discussion, engage others in the discussion, well-developed idea) and include at least one scholarly source.
  • One or two sentence responses, simple statements of agreement or “good post,” and responses that are off-topic will not count as substantive. Substantive responses should be at least 150 words.
  • I encourage you to incorporate the readings from the week (as applicable) into your responses.

Weekly Participation

  • Your initial responses to the mandatory DQ do not count toward participation and are graded separately.
  • In addition to the DQ responses, you must post at least one reply to peers (or me) on three separate days, for a total of three replies.
  • Participation posts do not require a scholarly source/citation (unless you cite someone else’s work).
  • Part of your weekly participation includes viewing the weekly announcement and attesting to watching it in the comments. These announcements are made to ensure you understand everything that is due during the week.

APA Format and Writing Quality

  • Familiarize yourself with APA format and practice using it correctly. It is used for most writing assignments for your degree. Visit the Writing Center in the Student Success Center, under the Resources tab in LoudCloud for APA paper templates, citation examples, tips, etc. Points will be deducted for poor use of APA format or absence of APA format (if required).
  • Cite all sources of information! When in doubt, cite the source. Paraphrasing also requires a citation.
  • I highly recommend using the APA Publication Manual, 6th edition.

Use of Direct Quotes

  • I discourage overutilization of direct quotes in DQs and assignments at the Masters’ level and deduct points accordingly.
  • As Masters’ level students, it is important that you be able to critically analyze and interpret information from journal articles and other resources. Simply restating someone else’s words does not demonstrate an understanding of the content or critical analysis of the content.
  • It is best to paraphrase content and cite your source.

LopesWrite Policy

  • For assignments that need to be submitted to LopesWrite, please be sure you have received your report and Similarity Index (SI) percentage BEFORE you do a “final submit” to me.
  • Once you have received your report, please review it. This report will show you grammatical, punctuation, and spelling errors that can easily be fixed. Take the extra few minutes to review instead of getting counted off for these mistakes.
  • Review your similarities. Did you forget to cite something? Did you not paraphrase well enough? Is your paper made up of someone else’s thoughts more than your own?
  • Visit the Writing Center in the Student Success Center, under the Resources tab in LoudCloud for tips on improving your paper and SI score.

Late Policy

  • The university’s policy on late assignments is 10% penalty PER DAY LATE. This also applies to late DQ replies.
  • Please communicate with me if you anticipate having to submit an assignment late. I am happy to be flexible, with advance notice. We may be able to work out an extension based on extenuating circumstances.
  • If you do not communicate with me before submitting an assignment late, the GCU late policy will be in effect.
  • I do not accept assignments that are two or more weeks late unless we have worked out an extension.
  • As per policy, no assignments are accepted after the last day of class. Any assignment submitted after midnight on the last day of class will not be accepted for grading.

Communication

  • Communication is so very important. There are multiple ways to communicate with me:
    • Questions to Instructor Forum: This is a great place to ask course content or assignment questions. If you have a question, there is a good chance one of your peers does as well. This is a public forum for the class.
    • Individual Forum: This is a private forum to ask me questions or send me messages. This will be checked at least once every 24 hours.

NU 665 C-Complicated Pregnancy Interactive

NU 665 C-Complicated Pregnancy Interactive

What following Questions do you have for Jamie?

  • How has being pregnant affected your emotional well-being and mental health?
  • Have you noticed any changes in your mood, anxiety levels, or stress since becoming pregnant?

As a PMHNP, what education would you provide Jamie about buprenorphine/naloxone use in pregnancy?

As a PMHNP or Psychiatric-Mental Health Nurse Practitioner, it is crucial to educate pregnant Jamie on the use of buprenorphine/naloxone during pregnancy. Buprenorphine/naloxone aids in stabilizing opioid dependence, preventing withdrawal symptoms in a mother, which can be detrimental to both the mother and the infant, and decreases the likelihood of recurrence of opioid use, which is a safer alternative to ongoing opioid abuse. Nevertheless, infants born to women who use buprenorphine are at risk of developing newborn abstinence syndrome (NAS). However, the NAS symptoms caused by this medication are generally less severe compared to other opioids (Link et al., 2020). Frequent expert monitoring is essential for the regular delivery of drugs in combination with regular examination of dose modifications.

Regarding Jamie, it is imperative that he attends all of his prenatal visits, maintains open communication with his healthcare team, and establishes a comprehensive strategy for managing the labour and delivery process in collaboration with them while also considering cases of Neonatal Abstinence Syndrome (NAS). Postpartum care encompasses several key components, including the need to change medication dosage, effectively address postpartum depression, and engage in discussions about nursing due to the potential transfer of medication into breast milk. Jamie should receive support and guidance throughout treatments, counselling, and participation in support groups (Link et al., 2020). These methods are effective in addressing the psychological components of a rehabilitation programme and in developing a long-term plan to prevent relapse. Faith in her husband’s ability to uplift her while maintaining a robust support structure would also be crucial for her general well-being.

What education can you provide to Jamie regarding her concerns?

You must keep taking the buprenorphine/naloxone prescription, Jamie. Relapse and a hazardous withdrawal phase might occur if you stop using this medication, which could be very harmful to you and your unborn child. Together with you, your obstetrician, and your anesthesiologist, we will devise a multimodal pain medication regimen to alleviate your discomfort to an adequate level. Although NAS is a concern, it is usually manageable, and when compared to other opioids, buprenorphine is an effective treatment. The best way to deal with your growing worry is to seek professional help, such as therapy or counseling. Also, it’s important to include your support system in your struggles to figure out what kind of emotional support you need (Link et al., 2020). Those efforts end up being quite useful. You may confidently and reassuringly handle this time of your pregnancy by being educated, participating in open communication with your healthcare team, and focusing on the positive aspects.

What screening tools would be appropriate to use with Jamie?

We suggest using the following screening tools to get a good look at Jamie’s mental symptoms and what’s causing them: using the GAD-7 to measure anxiety levels, the PHQ-9 to evaluate the degree of depression, and the PCL-5 to detect trauma-related symptoms of PTSD that may have developed as a result of the traumatic event during delivery.  In addition, the Edinburgh Postnatal Depression Scale (EPDS) can be utilized to identify depression throughout pregnancy. At the same time, the California-Suicide Severity Rating Scale (C-SSRS) is necessary to evaluate any indications of suicidal thoughts or actions in patients (Ahmadi et al., 2023). The equipment will assist the doctor in conducting a comprehensive assessment when diagnosing her mental condition.

Given your current information on Jamie, what other questions would you ask her regarding her current symptoms?

Following a mental health exam, I would inquire further about Jamie’s precise anxiety triggers and the frequency of her symptoms. Restlessness and anxiousness are two of them; it would be helpful to understand how they affect her day-to-day existence. Investigating potential triggers and recurrent intrusive thoughts, as well as how they affect the protagonist’s emotional world, can help uncover PTSD-related elements connected to the traumatic birth event. How minor depressive symptoms affect a person’s mood, day-to-day activities, and social relationships intrigues me. I’d also like to know if they’ve caused changes in eating patterns, energy levels, or social withdrawal (Link et al., 2020). The development of a thorough treatment plan intended to meet and address all of her mental health requirements throughout pregnancy will be aided by understanding how the indicators are affecting her functionality, the coping mechanisms she has attempted, her spouse, and other relevant parties.

List 3-5 differential diagnoses for Jamie.

Generalized Anxiety Disorder (GAD):

Post-Traumatic Stress Disorder (PTSD):

Adjustment Disorder with Depressed Mood

Major Depressive Disorder (MDD)

Of your differentials, which diagnoses would you, the PMHNP, choose as a primary diagnosis to work on for today’s session?

I will primarily focus on Generalized Anxiety Disorder (GAD) and Post-Traumatic Stress Disorder (PTSD), which have been classified as the main illnesses affecting Jim. The elevated scores she obtained on the GAD-7 and PCL-5 indicate that she exhibited pronounced symptoms of post-traumatic stress disorder and anxiety following her traumatic childbirth, subsequently affecting her daily functioning and emotional well-being (Link et al., 2020). The identification of these diagnoses involves examining potential triggers, devising coping strategies, and creating a contingency plan for the use of anxiety-reducing approaches and trauma symptom management throughout pregnancy.

It is essential to exercise increased caution when prescribing medication for her anxiety and PTSD. SSRIs, such as sertraline or paroxetine, are strongly recommended for their excellent safety profile and ability to inhibit serotonin reuptake selectively. These medications frequently emerge as the most optimal choice for women in terms of symptom management while posing the least amount of risk to the developing foetus. Prenatal exposure carries a risk, but postpartum withdrawal symptoms are typically minimal. Non-pharmaceutical treatments, such as cognitive-behavioural therapy (CBT), should be considered (Link et al., 2020). Medication selection should be influenced by trimester when considering the use of SSRIs during pregnancy. It is generally believed that SSRIs pose minimal or no hazards in the latter trimesters. However, patients need to be frequently monitored during all stages of pregnancy.

What patient education will you provide regarding your chosen medication?

I would provide Jamie with an educational session on the mechanism of action of the specific selective serotonin reuptake inhibitor (SSRI) pill she has chosen. I would emphasize its effectiveness in alleviating her anxiety and symptoms of post-traumatic stress disorder (PTSD) during pregnancy while also highlighting the importance of minimizing any potential risks to her baby. I would explain to the patient the significance of taking the drug in the correct dosage, as well as the possible adverse effects, such as nausea, dizziness, and sleep disruption. I would also provide the most effective ways to manage these side effects. The initial discussion should include a detailed explanation of the sequential progress made in alleviating symptoms and the significance of allowing sufficient time for improvement to occur (Serrell, 2021). Moreover, I will underscore the importance of regular follow-up sessions to monitor the mother’s mental health and the developing baby’s progress while ensuring that the medical professionals prioritize her emotional well-being.

What non-pharmacologic treatment options would you suggest for Jamie?

Jamie would consider cognitive-behavioural therapy, or CBT, to be effective because it doesn’t require taking medication. Any negative thought patterns that could lead to anxiety or PTSD symptoms as a result of the trauma she may have experienced during childbirth can be found and eliminated with CBT. Techniques like mindfulness meditation, law relaxation, and stress management are examples of skills that can enhance counselling programmes by offering practical ways to deal with anxiety and other stress-related issues that may arise during pregnancy (Serrell, 2021). Creating support groups for these pregnant women and providing them with a venue where they may hear the tales of others with the same condition and receive emotional support and encouragement from them could be another option to provide help to expectant mothers who have experienced trauma. The primary objective is to increase the person’s psychological resilience by teaching her coping mechanisms for any difficulties she may encounter and, ultimately, to give her the confidence to take care of her health without needing medicine.

What education do you provide to her?

Taking prescription sertraline in the form of SSRIs is safe for you and your unborn child in the second and third trimesters. It provides anxiety management without posing any dangers. As the performance progresses, the process management will monitor participant safety constantly. Since very little sertraline penetrates milk, it is unlikely that this medication will find its way into breast milk, which is why nursing mothers can use it. We’ll consider both the area’s advantages and any potential drawbacks (Serrell, 2021). After giving delivery, your next follow-up visit may be planned so that, should the need arise, we can resume your medication regimen. Maintaining contact with your medical team after giving birth is essential to voice any concerns and make sure you and your child are receiving the care you need.

What will be your safety plan and follow-up plan for Jamie?

Jamie might benefit best from therapies, including nonpharmacological drugs, such as cognitive-behavioural therapy (CBT). A psychiatrist may be able to assist Neya in managing her anxiety and PTSD symptoms by helping her recognize and alter her negative beliefs and actions. In addition to psychotherapy, practising stress management, mindfulness, and relaxation techniques will help you gain valuable skills to assist you in getting through the challenges of pregnancy. If other pregnant women were going through the same thing, they needed help. In addition, these networks of female support can inspire one another and offer beneficial peer interactions (Serrell, 2021). The techniques demonstrated are believed to assist Jamie in managing his mental health issues through nonpharmacological treatment of mental health issues and the development of a balanced lifestyle centred on medicine.

Please enter any additional information you would like to include in this case.

Developing a long-lasting partnership with Jamie’s team members is crucial to establishing her case, and two critical members of that team are her obstetrician and a psychotherapist who specializes in perinatal mental health. Her medical team will always be there to provide Jamie with care for the benefit of both mother and child if they monitor her mental health and medication response throughout her pregnancy (Ahmadi et al., 2023). Jamie’s ability to take charge of his care will be enhanced by training on medication safety, CBT substitutes, and networks that actively support individuals in making informed treatment decisions. These concerns would be crucial to ensuring that her pregnancy ends well and that she is prepared for the responsibilities of parenting since she is anxious about giving birth, experiencing PTSD episodes as her child grows, and the readjustment period that follows.

Write a 4–6 page annotated bibliography where you identify peer-reviewed publications that promote the use of a selected technology to enhance quality and safety standards in nursing.

Introduction

This assessment will give you the opportunity to deepen your knowledge of how technology can enhance quality and safety standards in nursing. You will prepare an annotated bibliography on technology in nursing. A well-prepared annotated bibliography is a comprehensive commentary on the content of scholarly publications and other sources of evidence about a selected nursing-related technology. A bibliography of this type provides a vehicle for workplace discussion to address gaps in nursing practice and to improve patient care outcomes. As nurses become more accountable in their practice, they are being called upon to expand their role of caregiver and advocate to include fostering research and scholarship to advance nursing practice. An annotated bibliography stimulates innovative thinking to find solutions and approaches to effectively and efficiently address these issues.

Professional Context

Rapid changes in information technology go hand-in-hand with progress in quality health care delivery, nursing practice, and interdisciplinary team collaboration. Technology is essential to the advancement of the nursing profession, maintaining quality care outcomes, patient safety, and research.

Preparation

Before you begin to develop the assessment, you are encouraged to complete the Annotated Bibliography Formative Assessment and select a Sentinel-U simulation to complete for practice. Completing these activities will help you succeed with the assessment. The Annotated Bibliography Formative Assessment will count towards engagement.

To successfully complete this assessment, perform the following preparatory activities:

  • Before you begin to develop the assessment, you are encouraged to complete the Annotated Bibliography Formative Assessment and select a Sentinel-U simulation to complete for practice. Completing these activities will help you succeed with the assessment. The Annotated Bibliography Formative Assessment will count towards engagement.
    • Direct patient care technologies require an interaction, or direct contact, between the nurse and patient. Nurses use direct patient care technologies every day when delivering care to patients. Electronic thermometers or pulse oximeters are examples of direct patient care technologies.
    • Indirect patient care technologies, on the other hand, are those employed on behalf of the patient. They do not require interaction, or direct contact, between the nurse and patient. A handheld device for patient documentation is an example of an indirect patient care technology.
  • Conduct a library search using the various electronic databases available through the Capella University Library.
    • Consult the BSN Program Library Research Guide for help in identifying scholarly and/or authoritative sources.
    • Access the NHS Learner Success Lab, linked in the courseroom navigation menu, for additional resources.
  • Scan the search results related to your chosen technology.
  • Select five peer-reviewed publications focused on your selected topic that are the most interesting to you.
  • Evaluate the impact of patient care technologies on desired outcomes.

NU 665 C Week 10 Geriatric Case Study

NU 665 C Week 10 Geriatric Case Study

Important missing information from the case study

For Midge, a comprehensive evaluation and treatment plan are only possible if some vital information is known. To start with, we do not know what specific medications Midge is taking now, nor do we have the dosages. This is important, particularly in light of her statement that her blood pressure readings are erratic, and chances are she may not be adhering to her prescribed hypertension medicine (as directed). In order to identify any drug interactions or side effects that may be causing symptoms of this kind, it would help to know the entire pharmacological treatment.

Drawing from the case study, it is evident that the outcomes of recent medical examinations or cognitive evaluations are absent. These assessments would provide significant information on Midge’s general health and mental abilities. Regular medical examinations might uncover latent health conditions that may impact her behavior, while cognitive evaluations would assist in assessing the degree of any cognitive deterioration.

Most often, families come from a common genealogical background, live in similar areas, and act in much the same manner. All together, these features may be indicative of inherited diseases within a family (Lobbia et al., 2019). This is so because doctors can tell if someone or their relatives are more prone to getting certain diseases by looking at recurring maladies among their close kin. It must be remembered that the case involving Midge does not reveal any familial history of dementia or mental illness for the subject. Such information could affect the identification of Midge’s risk factors and help with diagnostic decision-making. Such data is beneficial since knowing about hereditary basis plays a big role in many neurological and behavioral disorders (American Psychiatric Association, 2022).

Lastly, there is insufficient information on Midge’s eating preferences, nutritional condition, and sleep routines. In older adults, cognitive performance and general health may be adversely impacted by inadequate diet and disrupted sleep. Gaining insight into these aspects of Midge’s life would be beneficial in formulating a holistic care strategy.

Average developmental achievements and potential vulnerabilities

Midge is now 72 years old and in the late adulthood stage. Her stage in life comes with its spectrum of normative development and potential vulnerabilities. The bulk of the normative developments that would likely be on the ground at this level typically involve knowledge and experience accumulation. The majority of older adults like Midge have acquired a good deal of knowledge and expertise in life, and this may give them quite an advantage in making prudent decisions and problem-solving. Knowledge that could be an asset to this group as individuals and to the generations following them.

Grandparenthood and family bonds are often established throughout the latter stages of life. For several individuals, this phase of life comes with the pleasure of being a grandmother or, in Midge’s situation, a great-grandparent. Intergenerational interactions may provide individuals with a feeling of purpose and satisfaction. Nevertheless, Midge finds herself burdened by taking care of her great-grandchildren rather than finding it fulfilling.

Retirement is a milestone in the later years of life, particularly for older adults—a time when life offers extensive free time and the opportunity to pursue one’s interests. Midge has retired from working for Kroger, which could free up more leisure time for her. However, her current caregiving responsibilities appear to limit her ability to enjoy that aspect of later life.

While there are plenty of potential benefits to making it to late adulthood, there are also numerous perils to keep in mind as well. Physical and mobility concerns are at the top of most people’s lists. Weak muscles, difficulty in balancing, and long-standing health complications become common among older adults. If she were physically having problems, Midge would have more incredible difficulty handling activities in small children’s care.

Social isolation, therefore, is a risk that Midge is more likely to face when she reaches late adulthood. Reduced physical mobility, the loss of friends, and a social environment with few opportunities can create loneliness and feelings of isolation with detrimental effects on mental health.

Precipitating factors contributing to current symptoms

A lot of factors could contribute to the development of Midge’s symptoms. Taking care of young children at her advanced age is very likely to be among the most determinative factors. Looking after children aged 4 to 9 is a very demanding task from the point of view of energy and patience, as well as cognitively. An obligation of such nature could be too much for a 72-year-old woman and could add negative value to preexisting problems related to cognition or physical health.

Midge may be suffering from a grief response or residual trauma. The case study indicates that Midge’s husband died over ten years ago, and it speaks of a difficult childhood, where she had to stop schooling to be able to help out with her siblings’ needs when her father died. She has been a parent her whole life. Poor events in the past may have had lingering emotional impacts, now surfacing because of the stress of today’s realities.

Caregiving responsibilities can also lead to sleep disturbances, which might be another stressor. Good sleep plays an essential part in keeping the brain functioning normally and maintaining psychological stability. Lack of sleep because of late hours taking care of children could have a significant impact on Midge’s psychological well-being and ability to perform daily tasks or even cause hallucinations.

Differential diagnosis

One must consider alternative diagnoses with regard to Midge’s presentation. Alternative symptoms are consistent with dementia due to Alzheimer’s disease or other causes such as vascular dementia. Many of the potential dementias could be used to explain Midge’s symptoms, including what appears to be cognitive decline, difficulty with activities of daily living, and hallucinations. Due to her longstanding history of hypertension, it is essential to consider a specific workup for vascular dementia.

Another essential consideration is delirium. Unlike dementia, the development of delirium is typically acute and can be related to underlying systemic disease, drug side effects, or environmental factors. Midge’s confusion, hallucinations, and fluctuating changes in mental status could raise suspicions of delirium. Delirium has to be ruled out because it can often be reversed if the underlying etiology is identified and treated.

It is also essential to examine the possibility of a substance-induced psychotic condition, especially about probable interactions or adverse effects of medicine. Although there is no evidence indicating drug addiction, the combination of Midge’s blood pressure medicine and other medications she may be taking might result in cognitive changes or hallucinations.

Etiology of primary diagnosis

If we assume that the primary diagnosis is dementia, various things might contribute to its cause. Age-related changes in the brain primarily characterize dementia. As individuals age, the brain naturally experiences changes, such as shrinkage (atrophy), decreased blood supply, and inflammation. These alterations might potentially lead to a decrease in cognitive function and raise the likelihood of suffering from dementia.

Given Midge’s history of hypertension, vascular problems are most certainly a significant factor in her condition. Damage to brain blood arteries from chronic hypertension may limit blood flow and even cause cell death. A major cause of vascular dementia, this vascular injury may exacerbate cognitive impairment. The state of the blood vessels is essential in how symptoms develop in all types of dementia, including Alzheimer’s disease.

Midge may be genetically predisposed to dementia. Though there is no information regarding family history, it is pretty common that many types of dementia have genetic heredity. In Alzheimer’s disease, specific genetic mutations increase the risk for this type of dementia.

The effects of chronic stress on brain health also need to be considered. Early family obligations, her husband’s death, and her present caregiver responsibilities are just a few of the many stresses Midge has had to overcome throughout her life. An increase in cortisol levels caused by chronic stress may eventually cause damage to the hippocampus, an area of the brain that is essential for learning and memory.

Monitoring and assessing physiologic complications

Regular physical checkups may detect additional health concerns causing Midge’s symptoms. Many health issues might affect cognitive performance in older adults; thus, systems should be thoroughly reviewed.  In Midge’s case, regular cognitive tests are needed. MMSE and MoCA are standardized cognitive function tests. These tests may follow cognitive changes and guide therapy.

Vitamin shortages and thyroid issues may be ruled out with blood testing. Thyroid problems and vitamin deficiencies (especially B12 and D) might manifest as mental fogginess. To further understand Midge’s health, doing a metabolic panel, complete blood count, and vitamin level testing would be helpful.

There is a dire need to monitor Midge’s everyday life skills. Regular health provider evaluations or family reports may help achieve this. Midge’s health deterioration may be tracked by changes in her capacity to care for herself or the children.

Useful nonpharmacologic therapies

Midge may benefit from a variety of nonpharmacologic treatments. People with mild to moderate dementia or Alzheimer’s disease may benefit from cognitive stimulation treatment (CST), a brief intervention program that is based on research. It can be done either individually or in groups. Dementia patients may maintain their cognitive abilities and social engagement with the help of CST, which consists of a sequence of themed activities. A therapist may help Midge by introducing activities that stimulate the mind and encourage social interaction, such as memory games and puzzles.

Midge could surely benefit from reminiscence therapy. Reminiscence therapy is a form of psychotherapy that involves thinking or talking about past experiences. By focusing on those past events, individuals experience satisfaction. Often, its applied among older adults, meaning it could serve an individual with overall well-being. Since Midge is attached to such traditional hymns, this can really be comforting and interesting therapy for her.

It is worth considering music therapy, mainly if it includes Midge’s favorite hymns. Music therapists employ music components such as tone, rhythm, and harmony to achieve stress reduction or enhance quality of life (Bakar et al., 2021). For Midge, listening to music she knows and loves might be a source of solace and inspiration.

Helpful medications and reasons

While nonpharmacologic treatments are essential, several drugs may ease Midge’s symptoms. Alzheimer’s disease and other dementias are treated with cholinesterase inhibitors such as donepezil, rivastigmine, and galantamine (Redulla, 2019). These drugs boost acetylcholine, a neurotransmitter important in memory and thinking. In early to middle dementia, they may enhance cognitive performance.

Antidepressants may be helpful in cases when depression is present. It is believed that antidepressants alleviate depression by elevating levels of certain neurotransmitters. Certain brain chemicals may enhance mood and emotion. Due to their comparatively benign side effect profile, older individuals generally choose SSRIs like sertraline or escitalopram (Bakar et al., 2021). Improving cognitive performance and quality of life may be possible by treating depression if it is present.

It is essential to ensure the effective control of Midge’s hypertension. Individuals who have high blood pressure are advised to adopt healthy lifestyle modifications. The recommendation of medication is contingent upon an individual’s blood pressure measurement. Adequate blood pressure control is crucial for mitigating the risk of further cognitive deterioration in Midge, mainly if vascular factors influence her symptoms.

Safety risks and treatment plan

There are some safety concerns going on that the team will have to address with Midge. Wandering is one safety concern that needs to be addressed immediately. During the assessment, she walked with the children down the unpaved road. Not only is this type of behavior a risk to Midge herself but also to the children for whom she is responsible. The treatment plan for Midge could include adding door alarms and a GPS locator to address this specific potential safety concern. This function would be able to alert Midge’s daughter or other carers if Midge left the house and forgot to tell them.

Falls are common health issues among the aging population. Falls among older individuals with hypertension may compromise their general health and well-being (Rhoads, 2021). For that matter, fall risks should be reduced, especially at night when Midge checks the woodpile. A treatment plan should also allow for a thorough review of home safety conditions; these can include better lighting, removal of items that may cause stumbling, and handrails where necessary. It would be advisable for Midge to use walking aids where appropriate, as these may reduce the risk of falling.

Module 2 Discussion

DQ1 How can conflict in the workplace affect the quality of care for the clients? Describe examples and techniques you have seen.

DQ2 1. How do you define quality of care?

2. What factors can you control, as a manager, for your staff to provide high-quality care (which factors are internal and which are external)?

3. If an organization is deciding between several quality management programs which would you recommend and why?

Quality Management Programs

BENEFITS OF QUALITY MANAGEMENT SYSTEMS
Implementing a quality management system affects every aspect of an organization’s performance. Benefits of a documented quality management system include:

Meeting the customer’s requirements, which helps to instill confidence in the organization, in turn leading to more customers, more sales, and more repeat business
Meeting the organization’s requirements, which ensures compliance with regulations and provision of products and services in the most cost- and resource-efficient manner, creating room for expansion, growth, and profit
These benefits offer additional advantages, including:

Defining, improving, and controlling processes
Reducing waste
Preventing mistakes
Lowering costs
Facilitating and identifying training opportunities
Engaging staff
Setting organization-wide direction
Communicating a readiness to produce consistent results
ISO 9001:2015 AND OTHER QMS STANDARDS
ISO 9001:2015 is the most recognized and implemented quality management system standard in the world. ISO 9001:2015 specifies the requirements for a QMS that organizations can use to develop their own programs.

Other standards related to quality management systems include the rest of the ISO 9000 series (including ISO 9000 and ISO 9004), the ISO 14000 series (environmental management systems), ISO 13485 (quality management systems for medical devices), ISO 19011 (auditing management systems), and ISO/TS 16949 (quality management systems for automotive-related products).

ELEMENTS AND REQUIREMENTS OF A QMS
Each element of a quality management system helps achieve the overall goals of meeting the customers’ and organization’s requirements. Quality management systems should address an organization’s unique needs; however, the elements all systems have in common include:

The organization’s quality policy and quality objectives
Quality manual
Procedures, instructions, and records
Data management
Internal processes
Customer satisfaction from product quality
Improvement opportunities
Quality analysis

Lesser-Paid Employee NURS 4455 Discussion

Explanations for Pay Disparity

There are often legitimate reasons for treating the compensation of two workers differently.

Education may be one consideration: One worker may hold a certification or advanced degree that the other doesn’t, and that could justify higher pay.

Experience is another factor: An employee who has worked at a company for 10 years may earn less than one who was just hired—even if they are performing the same job duties—because the new hire already put in 12 years at a previous company.

The complainant in the SHRM online discussion, however, had been at the company for several years but was being paid $5,000 less a year than a new hire recently out of college—someone the established employee had to train.

[SHRM members-only platform: SHRM Connect]

In a case like this, some questions that line managers or HR managers may want to ask themselves about the two workers to ensure the pay disparity is fair and legal, Ocasio said, are the following:

Are the two positions really exactly the same?
Has the position of the lower-paid employee changed or evolved since she was hired?
Were there times when the lower-paid employee went above and beyond the call of duty?
What is the comportment and attitude of the lower-paid employee?
